Description
Case Management for adults with intellectual/developmental disabilities who receive services through the HCBS Choices Waiver. Case Managers facilitate person-centered planning and assist people in applying to service providers and community resources. Serves residents of the Northeast and Southeast Regions in South Dakota. Conflict-Free Case Managers are not related to the individual, are not their paid caregivers, and are not financially responsible for the individual.
