Eligibility

-Must live in the United States -Must be at or below 250% of federal poverty level -Have an eyeglass prescription dated within the last 24 months -Have no other resources available to pay for glasses, including federal/state programs

Application process

Best if submitted online by a social worker, case worker, or an agency. Individuals who do not have access to an agency may apply directly but must provide proof of eligibility and will be charged a $10.00 program fee. This application is only for individuals who do not have a social worker, a case worker, or other health advocate. Visit website for individual application.
