Child Medical Applicants: Must be 17 years of age or younger, have a confirmed medical appointment requiring air travel, have a departure date of at least 14 days away or longer from the date of the application, be medically cleared to safely travel on a commercial airplane, and have income below the financial guidelines. Service Dog Applicants: open to all ages, must have a confirmed service dog retrieval or training appointment scheduled requiring air travel, have a departure date of at least 14 days away or longer from the date of the application, be medically cleared to safely travel on a commercial airplane, and have income below the financial guidelines. Financial eligibility is based on household size and is generally three times the listed annual Federal Poverty Income guidelines listed by the U.S. Department of Health and Human Services.
