Application process
Only accepts applications from social service professionals sent in by U.S. Mail or E-mail. As soon as all documentation is gathered, the case will be decided at the weekly Disbursement Committee meeting.
Eligibility
Professionals (caseworkers, case managers, health care workers, social workers, counselors, teachers, clergy) who apply on behalf of their patients and clients needing mobility equipment, minor home modifications for handicap accessibility, ramps, prosthetics, etc.
