Fill out a NAPIS form at either Active Generations location or call the Nutrition Department to register.
Meal cards can be purchased at both front desks at each Active Generations location. Each punch on the card is $6.00. Cards can be modified to how many meals you want to purchase.

Description
Hy-Vee and Active Generations Nutrition Department partner together to offer "Your Choice Meals" - a hot nutritious meal for adults aged 60 and over, their spouse, and/or dependent adult living with them. The Hy-Vee "Your Choice" card offers the option of presenting pre-paid card at the Hy-Vee deli counter and selecting a meal from a designated menu. The diner can choose one meal per day. Sample Menus can be found on the website If interested, individuals are required to fill out a short National Aging Program Information System (NAPIS) form. The information is kept confidential.
