Improves how South Dakota connects veterans and military families to resources they need such as mental health support, basic needs, and social care. Focuses on care coordination and early access by strengthening how community and public partners work together, especially when individuals are not engaged in care. Supports shared communication, reduces delays, closes gaps between providers, and improves follow-through between organizations involved in care.
