Eligibility

Open to all individuals who have a confirmed diagnosis of a serious health condition or the patient must be getting testing for the condition, be in active treatment for the health condition or starting treatment in the next 60 days or the patient must have finished treatment in the last 6 months. Must be a U.S. citizen or a permanent resident of the United States and be getting treatment in the United States or a U.S. territory.

Offers one-on-one professional assistance with eliminating healthcare access barriers for individuals living with a serious health condition. Provides assistance with accessing prescribed medical treatment and services, understanding health insurance benefits, paying for treatment, paying for living expenses, and employment during medical treatment. *Providing a quality patient experience to every patient we work with is our number one priority. To ensure our case management team can provide the in-depth, hands-on support required to resolve access and affordability challenges faced by patients and families calling us for help, we limit the number of new cases our case management team can accept each day.
