Residents of the Northern Black Hills, (ages 19-64) who are underinsured and uninsured (must not qualify for Medicare, Medicaid, Veterans Assistance) and meet income guidelines apply.

Application process

Walk-in, no appointments accepted. Required to visit with an eligibility volunteer to review the financial guidelines for the clinic, which are based on 200% of the Federal Poverty Guidelines. Registration is from 6:00pm - 7:00pm.
