Application process

Walk-in, no appointments accepted. Required to visit with an eligibility volunteer to review the financial guidelines for the clinic. Registration is from 6:00pm - 7:00pm.

None (Patients referred for further medical tests will need to complete additional financial information paperwork).

Healthcare and emergency dental care to financially eligible residents of the northern Black Hills, ages 19-94, regardless of health insurance status. The Lead clinic provides medical services only. Patients at the Lead clinic may visit the Spearfish location for dental services. Referrals are made for diagnostic imaging or radiology, general lab tests and flu vaccines. ​​Works with 7 pharmacies in Belle Fourche, Deadwood, Lead, Spearfish and Sturgis.
