An in-home rehab program providing a high level of coordination, support, communication, and follow-up with the goal of supporting the client in an independent setting. Services are tailored to the clients needs.
This includes accompany clients to their doctor appointments, explaining their insurance explanation of benefits, pharmacist review of medication, and having access to resources to coordinate hospital discharge (i.e. necessary medical equipment, new medications, transportation, and follow-up appointments and services).
