Open to individuals 55 years of age and older who live in Bon Homme, Charles Mix, Douglas and/or Hutchinson County, and to caregivers.

Description

The goal is to help older adults and caregivers in Bon Homme, Charles Mix, Douglas, and Hutchinson counties access services and supports to successfully age at home. The program is open to help with identifying resources for many needs and focuses on transportation, food security, social isolation, safe housing, and healthcare navigation/referrals.
