Team Approach Hospice Care is delivered through a coordinated, team-based model that includes medical director, nurses, home health aides, social workers, spiritual counselors, therapists, and trained volunteers. This team works collaboratively to address the full spectrum of a patient's and family's needs-medical, emotional, social, and spiritual.
Care in the Patient's Preferred Setting Hospice services are flexible and can be provided in various settings, including the patient's home, assisted living communities, or skilled nursing facilities. This enables patients to remain in a familiar and supportive environment surrounded by loved ones.
Support for Family and Caregivers: Comprehensive guidance and emotional support for family members, including caregiver education, respite care, counseling, and bereavement services to help families navigate caregiving and cope with loss.
