Horizon Health - Wessington Springs Medical is a full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Wessington Springs Dental provides a full range of family oral health services including preventative care (check ups, fillings), restorative and cosmetic dentistry (crowns, bridges) , endodontics, and oral surgery (root canals, extractions).

Rapid City utility customers can apply for a reduction of their monthly utility charges through the Utility Rate Relief Program. The program is designed to lower the monthly utility expense for senior citizens and people with disabilities. If qualified, the rates charged for all utility services billed to the applicant will be reduced by 25 percent for a period of two years or until the renewal date established by the Public Works Director. Applications are accepted at any time, but renewal requests must be received by April 30 of odd numbered years for participants to remain on the program.

GROW South Dakota provides emergency assistance to help individuals/families pay for utilities, or other emergency needs. Assistance is limited to those in the service area who have not received funds within the last year. Income and program guides may apply. Availability of funding is extremely limited; funds may not always be available.

Provides information about payment assistance and low monthly payments for those needing help purchasing hearing aids. Monthly costs range from $0 - $49.00.

This office is the keeper of County records and acts as a bookkeeper of County business. The Auditor is responsible for County Commissioner meetings including scheduling, agenda, business follow-up, Preparing the County's budget, Audit bills or other claims against the County and prepare the warrants in payment of these bills, Figuring the mil levy, and Head election officer of the County. Also deals with Voter Registration and Absentee Voting. The Auditor's Office also handles requests for one-time economic assistance that may include Rent, Utility for current bills, May be available for water, lights, heating, Medical, hospitalization, and burial assistance requires pre-approval if not for emergency services. All other resources must have been exhausted first. Assistance does require payback and county lien is filed until paid in full.

ICAP Housing Services provides the following programs: Rapid Rehousing and Homeless Prevention Programs: aims to help two specific groups - persons who are at great risk of homelessness and those who have become homeless. Services that can be provided include case management, security deposit and rent assistance. These services are aimed at helping participants to retain their housing, therefore avoiding homelessness, or to re-enter appropriate permanent housing as quickly as possible. Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Homeless Rent and Deposit: Helping homeless households get into housing. Must provide steady and reliable source of income. Applicant must agree to case management services.

Patients can set up a payment plan by calling Patient Financial Services. Sanford Health also offers Financial Assistance for emergency and other medically necessary services provided and billed through centralized Patient Financial Services. Patients can call to request an application, or can visit the registration representative at the clinic or hospital and they will be able to provide an application. The patient will need to return the application completed in its entirety along with: Copy of most recent complete tax return, including all schedules. Copies of the two most recent pay stubs for each wage earner in the household. Copies of property tax assessment statements Applications will be reviewed and a decision will be communicated within 30 days. All information is confidential. Behavioral health services include outpatient individual, couples, and family counseling, psychological assessment and evaluations (non-court ordered), and case management.

Provides the following services - Individual therapy, Marriage and couples therapy, Family and group therapy, Evaluations (psychological and psych-social), Patient education, Case management services, and Emergency Mental Health Services available 24 hours a day / 7 days a week. Works in partnership with the Tribal Community to raise their physical, mental, social and spiritual health by providing timely caring services.

Assists with the following as a last payer - indigent medical claims (excluding prescriptions), utilities, and burial, and funeral expense assistance.

County provides assistance to individuals at or below the federal poverty guidelines for funerals and emergency medical bills.

Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Elder Angel Funds are available through a short application process to help low-income senior citizens with everyday expenses such as utilities, groceries, clothing, medical co-pays, prescriptions, etc.

A state agency that works with individuals who are blind and or visually impaired in the area of employment and independent living. Services that are available if eligible Vocational counseling and evaluation, Job placement, Training in specialized skills of blindness (cane, travel, braille, daily living), Job development, On-the-job training, Job follow-up, Assistive technology services, Job retention, Funding for post-secondary training or eye surgery (If economic need is met), and Low Vision services (includes a visual examination conducted by a trained specialist in the field of low vision, assessment of visual functioning of individual needs and goals, prescriptions for specialized low vision devices, and individualized training).

Eligible participants may receive a voucher, which is used to purchase garden seeds and/or live plants which helps to ensure that the participating households have access to fresh fruits and vegetables, which helps them to maintain nutritional well-being. If funding allows, the participants can also purchase canning or preserving supplies so that they can save some of the produce for use in future months.

Sioux Falls Parks and Recreation offers a variety of admission options for purchasing swim passes, whether you need a one-day pass or a swim pass for an entire year. Renewing a swim pass: Keep your key fobs from year to year, as key fobs can be renewed each year. Replacements of a lost key fob are available for $3.00 at the Midco Aquatic Center. Online renewal available online (for full price passes). To purchase a new season swim pass: Create an online account OR Visit the Midco Aquatic Center or an open outdoor aquatic facility to have photos taken of immediate family members for key fobs. Free or reduced priced swim passes: Summer season (May - August) are available for qualified families (immediate family members living in the same household, including youth age 18 and younger, stepchildren or foster children. Only two people over the age of 18 in the household can be on the swim pass) who are residents of Sioux Falls currently enrolled i the SNAP program (food stamps). If you qualify for a reduced or free pass, you must visit the Midco Aquatic Center or outdoor aquatic facilities and provide qualifying documentation, which includes: Photo ID Dakota EBT card (food stamp card) Proof of residence/physical address (must contain your name and current address). Examples include utility bill, credit card statement, pay stub/earnings statement, rent receipt, phone bill, tax document, transcript from an accredited school, bank statement, or any other certified mail. If a Sioux Falls resident presents an EBT or Medicaid card, their immediate family can get in for $1.00/person (only available during the months of September - May).

Offers assisted housing, public housing authority, security deposit and rental assistance for the elderly, people with disabling conditions, and families meeting low-income guidelines. Does not offer emergency housing assistance. Families pre-apply and are placed on the waiting list. As opening become available families are contacted. Families are given a voucher to rent with in the community or rent an apartment or home from HRCP. Yearly qualifications are required from the tenant to maintain tenancy. The rent payment assistance has a waiting list and is not an emergency rent payment assistance option.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides transitional and supportive housing services to veterans who are homeless or at risk of homelessness due to financial hardship, unemployment, addiction, mental health, or transition from jail. Services include connection to immediate food and shelter needs, job training, life skills development and education, justice system navigation for community reentry from jail, and financial support to prevent homelessness.

Offers services to eligible voters. Registration of new voters, updates to existing voters and verification of voter data. Administers elections on behalf of the county. Point of contact for the county commission, serving as their clerk during meetings and in compiling data for their use. Arranges the agenda and appointments. Administers the county level indigent care for those who can not afford treatment. Specific criteria is in statute for what is eligible and ineligible. Manages the mental health budget for when people receive treatment but can not afford to do so on their own. Programming is contracted with Community Counseling Services of Huron.

Provides assistance with rent (does not require eviction notice; cannot assist with deposit) and utilities.

Arranges no-cost plane transportation within the continental United States for cancer patients going to and or from recognized cancer hospitals by using empty seats aboard business aircraft operating on business flights.
