Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Services provided include oxygen, ventilators, apnea monitors, CPAP (continuous positive airway pressure), nebulizers, oximeters, walkers, crutches, canes, wheelchairs, lift chairs, medical alert systems, power mobility services, and other assistive living devices, breast pumps, phototherapy, mastectomy garments, ostomy supplies, compression garments, pumps, wound supplies, and orthopedic bracing. Infusion services also available. Professional staff include: RT's, LPN's, Pharmacists, ATP's, and other trained professionals are on staff to assure high quality services are provided.

Long Term Services and Supports provides opportunities to enhance the quality of life for older adults, adults with disabilities, and their caregivers through home and community based services. Services Include: home care services to help adults remain in their home, safety and help moving around, nutrition and meals, Senior Health Information and Insurance Education (SHIINE) providing individuals information about benefits they can receive through Medicare, caregiver support, Structured Family Caregiving, residential living services, Ombudsman program providing advocacy and education for residents, family, and staff in long term care settings, and adult protective services providing interventions to protect vulnerable adults from abuse, neglect, and exploitation.

Sells new and gently used furniture, appliances, building materials and more to the general public at a fraction of retail prices. The items are donated by individuals, corporations, businesses, and contractors. Profits support local individuals and families building stability through Habitat homeownership and also keeps tons of reusable material out of the landfill each year.

Customers can use Personal Emergency Response Systems (PERS) inside and outside of their homes with GPS tracking. Fall detection is available for all patients when needed.

Provides home care and nursing services to anyone 18 years or older who needs assistance with activities of daily living (ADLs) due to illness, injury, disability, or aging. Services take into consideration spiritual, physical and emotional needs. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

 Offers low interest rate loans, deferred loans and forgivable loans to assist with repairing or replacing home's electrical, plumbing, furnace, water heater, roofing, decks, steps, windows, doors and other life safety issues. Eligibility for the programs are determined by income, and family size (based on county of residence). HUD certified counselors can help people walk through this process, from start to finish. Home must be on a permanent foundation to qualify for program. Manufactured homes on rented lots are not eligible properties. Eligible properties must be owner occupied. The property must be located on a permanent foundation, and the occupant must own the land or have qualifying land lease.
