A primary care, family medicine clinic that provides comprehensive care for the entire family through all phases of life, from childhood to senior years. Board certified physicians and physician assistants offer health care services sensitive to individual needs. Provides services for adult patients who have severe and persistent mental illness. The goal is to assist the person in developing skills that will help them live in the least restrictive and most normal setting possible. The programming is designed to help patients develop life skills that will be needed for discharge placement and to restore patients to competency for court.

In-Shelter or Out-of-Shelter services provided to victims and survivors of domestic violence and sexual assault, and their families, so they can make decisions about their lives in a supportive, non-judgmental and empowering atmosphere. Ongoing advocacy is provided and assistance with obtaining resources to seek, or rebuild, a new and safe life. Provides courtroom advocacy, assist with protection orders, can provide interpretive services, and safety planning, and can assist with supplies for day-to-day living and setting up a new household. Children are welcome. Assistance with transportation needs can be addressed. Shelter can accommodate nine adults and several children.

Offers individual, couples and family therapy in both English and Spanish. Services include dialectical behavioral therapy, trauma informed therapy, cognitive behavioral therapy, EMDR therapy and counseling for LGBTQ+, child abuse and child abuse survivor counseling for adults, grief and bereavement counseling, terminal illness counseling, and gender identity counseling.

The Union County Veterans Service Office (VSO) educates and assists veterans and their dependents and/or survivors in applying for veterans' benefits including benefits from the Department of Veterans Affairs and the State of South Dakota. Referrals are also made to other agencies that may have potential benefits for veterans. Areas of primary activity are obtaining military records and discharges, death/burial benefits, education benefits, home loan certificates, non-service connected pension, service connected compensation, state bonus program, survivors benefits, and veteran affairs health care. Veterans are encouraged to register their discharge information (DD Form 214, Report of Separation) with the Union County Registrar of Deeds Office.

The Mitchell office only provides Vocational Rehabilitation Services. Voc. Rehab Counselors can make referrals to the above services and programs if clients reside in the Mitchell area.

Provides equine-assisted counseling conducted in an arena with a team including a mental health professional, an equine professional, and one or more horses for individuals, couples, families, or groups. Specializing in Post Traumatic Stress Disorder (PTSD), moral injury, trauma, addiction, grief and loss, abuse, depression, anxiety, and mood disorders. Team building and professional development workshops are also available.

Life Coaching Services provided to adults and children above the age of 8 via Zoom.

Specializing in food and weight issues; areas of expertise covers the full spectrum of disordered eating. Focuses on compulsive / emotional eating / binge eating disorder and weight loss coaching. Services provided include individual counseling / coaching, groups, and personalized small groups, classes, and workshops. Offering telehealth services. Prefer first session in person.

Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Offering compassionate and comprehensive care for all patients. Services include annual physicals, immunizations, medication management, transitional care management, joint injections, lesion/wart removal, splinting/casting, and Behavioral Health appointments via telehealth. Same Day Appointments are available during regular business hours Monday to Friday as well as Urgent Care appointments on Saturdays.
