A joint effort of the Scottish Rite Foundation of South Dakota and the Communication Sciences and Disorders Department at USD with support from the Sioux Empire United Way. The clinic's purpose is to provide evaluation, treatment, and outreach services for children birth to 21 years in the Sioux Falls area with speech, language, and literacy deficits. Also provides a training site for graduate students in Speech Language/Pathology. Examples of disorders seen include children who are late talking, who have articulation disorders, language disorders, stuttering difficulties, hearing losses, etc. The clinic also serves as a diagnostic center for written language / dyslexia. There is a minimal fee for services; no one is denied due to an inability to pay. Referrals are taken from parents, professionals, and family members.

A full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Provides drug and alcohol treatment for adolescents ages 13 - 17 years of age who are enrolled as a member of a federally recognized Tribe or able to show the lineal descendancy. It is a rolling admission and can be anywhere from 90 - 120 days.

This means that if a patient is enrolled in a federally recognized tribe or can prove lineage of descent of an enrolled tribal member; the patient may receive direct care services at Oyate Health Center. Prescription Assistance - Any Native American enrolled in a federally recognized tribe can receive prescription expense assistance at any IHS facility.

Offers the following services: primary care, birth control, morning-after pill (emergency contraception), breast and chest exams including mammogram referrals if necessary, HIV testing, LGBTQ Services, men's health care, pregnancy testing and services, STD testing/treatment/vaccines, women's health care, gender affirming hormone therapy (GAHT), HPV vaccinations, Hepatitis B vaccinations, and Flu shots. Services take into consideration spiritual, physical and emotional needs. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Goes to child centers and screens children (age 3 - 6 primarily) for speech and language development. If children do not pass the screening, recommendations are made for appropriate action. Only those children whose guardians sign them up are screened.

Services include medical assistance such as in-home memory care, diabetes care, arthritis care, respite care, Alzheimer's and Dementia care, hospice support, chronic illness care, and 24-hour care. They can also assist with personal care (i.e. bathing, dressing), companionship, assistance with daily activities (i.e. meal preparation, light housekeeping), and transportation. These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.
