The IMPACT (Individualized and Mobile program of Assertive Community Treatment) program is designed for individuals with severe mental illness who are in need of intensive supports in the community. Services are highly mobile and delivered in the clinic, home or other community-based locations depending on need and consumer preference. Treatment is based on a comprehensive assessment and the patient is involved in each step of treatment planning. IMPACT services may include symptom monitoring, medication management, nursing care, individual or group counseling and education, direct assistance in obtaining basis necessities and other supports. Services are provided by an interdisciplinary team of professionals that include psychiatrists, psychologists, advanced practice nurses, mental health counselors, psychiatric social workers and case managers.

Provides pharmacy services such as OTC medications, immunizations, long-term care, diabetic care, vitamins and supplements, and is a medical disposal take-back site. Customers can download the PocketRx mobile app to set medication reminders and refill medications 24 hours a day/7 days a week.

Services include skilled nursing, physical therapy, occupational therapy, speech therapy, personal care, dressing, medication assistance, lab draws, and home infusion therapy. Also specializes in providing home care after surgery.

Provides independent living and assisted living services. Independent living services include nutrition services, weekly housekeeping, life enrichment programming, and 24-hour on site staffing. Assisted living offers a more supportive environment offering 24-hour personal care services including nutrition services, housekeeping, life enrichment programming, and personal assistance with activities of daily living. Also provides memory care services.

Provides assisted living care to those who can no longer remain alone in their home. Services include but are not limited to medication administration, dietary services, laundry and housekeeping services, 24-hour care, daily activities, assistance with activities of daily living, and security options.

Pharmacy Services include private consultation, medication therapy management services, home and mail delivery, durable medical equipment, medication synchronization, compounding, over the counter medications, and first aid supplies.

Provides assisted living care including the following services: assistance with activities of daily living, prescription assistance, in -room call system, and weekly nursing care.

Horizon Health - De Smet Medical is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT).

Providing inpatient-level care for individuals who can safely receive care in the home. This consists of visits with a Registered Nurse, Advanced Practice Provider and Physician visiting in person or virtually.

Services are provided within the patient's home. Includes non-skilled nursing care (e.g. foot care, general assessment, medication management, colostomy care, etc.), personal care (e.g. bathing, hair care, oral/mouth care, exercises, and medication reminders), and housekeeping (e.g. vacuuming, dusting, kitchen/refrigerator cleaning, bathroom cleaning, laundry, etc.).

Offers a family-centered approach to care and is committed to providing only the most appropriate services based on regular assessments of your needs. Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Provides home care and nursing services to anyone 18 years or older who needs assistance with activities of daily living (ADLs) due to illness, injury, disability, or aging. Services take into consideration spiritual, physical and emotional needs. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

Offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Isabel is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

Clients can utilize personalized care to tailor their wants and needs and provide in-home assistance. Services include medication management, toenail care, home assistance, and assistance with activities of daily living (ADL). Also takes part in the South Dakota Department of Human Services Homemaker, Personal Care and Nursing Services.

Provides assisted living care including the following services: 24-hour staffing, dietary services, medication administration, recreational activities, laundry and housekeeping, emergency call systems, assistance with activities of daily living, and coordination of care needs.

Services provided include prescription pick-up (these are delivered from Freeman daily), diabetes education and support, durable medical equipment (i.e. canes and walkers, bathroom equipment, orthopedic supplies, nebulizers, etc.), immunizations, long-term pharmacy, Medicare open enrollment, and a medication disposal site. Accepts UPS drop off packages for free. Customers can use the My Community Pharmacy to refill medications, set medication reminders, and track refill status.

Outpatient Services - Individual therapy (for mental health or substance use), Family therapy, Group therapy, Medication management, Psychological assessments and testing, Commitment evaluations, Nursing home consultations, and 24-hour a day crisis intervention via crisis line. Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Horizon Health - Huron is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, immigration physicals, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Horizon Health - Huron also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Spanish and Karen translators available on-site.

Home Health: Offered skilled home health services including nurses, physical therapy, occupational therapy, speech therapy, and home health aides providing care, support and education to allow patients to manage health conditions at home. Hospice: Offers a multidisciplinary team that addresses the patient and their family addressing physical, psychosocial, spiritual, and bereavement needs. Services include clinician on-call 24/7, pain management, guidance with Advanced Care Planning, forms, and financial care needs, provide medications, durable medical equipment, and supplies needed to treat life limiting illness, support and teach family/caregivers how to care for the patient, and grief/bereavement counseling for up to 13 months after the patient has passed away.

Offers skilled nursing services, homemaking, personal care, bathing assistance, and shopping assistance.
