Provides in-house services and works closely with state and community stakeholders for warm-handoffs and referrals such as counseling, primary care needs, psychiatry, food, housing, employment and financial assistance. An in-home rehab program providing a high level of coordination, support, communication, and follow-up with the goal of supporting the client in an independent setting. Services are tailored to the clients needs. This includes accompany clients to their doctor appointments, explaining their insurance explanation of benefits, pharmacist review of medication, and having access to resources to coordinate hospital discharge (i.e. necessary medical equipment, new medications, transportation, and follow-up appointments and services).

Providing in-home care to those who need assistance with activities of daily living. Services include medical assistance such as in-home memory care, diabetes care, arthritis care, respite care, Alzheimer's and Dementia care, hospice support, chronic illness care, and 24-hour care.
