Provides nursing facility and short-term rehabilitation care in a home-like environment with individualized care plans. Services include but are not limited to 24-hour nursing care, wound care management, nutrition services, safety systems, pain management, recreational activities, in-house pet therapy, physical/occupational/speech therapy, and restorative therapy. Also provides memory support, hospice, respite care services.

Provides psychiatric services to adults diagnosed with a severe mental illness who are in need of case management and/or medication management. The transitional housing is an 11 bed facility with private rooms that provides supportive services to individuals who are homeless or currently residing in an unstable environment who have a substance use disorder as well as a severe mental illness. This is a sober living environment. Residents are provided 2 meals per day during weekdays and 3 meals per day on the weekend. Residents are expected to participate in constructive daily activity outside of the transitional facility 9:00am - 3:00pm, on weekdays. Life skills groups and on-site laundry also provided.

A for-profit retail pharmacy providing prescription management, refills and transfers plus over the counter drugs. Also serves as a medication disposal site, with drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses.
