Provides nutritious meals, companionship, and community support to seniors and individuals in need while empowering them to live independently with dignity in their homes. Consists of a hot meal delivered to their door Monday through Friday from 10:45am to 12:30pm. Menus meet 1/3 of the Dietary Reference Intakes (DRI's) for the population served. Menus are designed by a Registered Dietitian and are low in fat and sodium. Special diets are available upon request; diabetic, gluten free, renal, as well as textured modified servings. Supportive services include: information and referrals, nutrition counseling and education.

The Health Cooperative is here to partner with patients to stay healthy and meet their health goals. An inter-professional team is available to offer early evaluations of adult health concerns. Also provides personalized self-care education that will assist patients in making healthier choices. This is not a replacement for a primary care doctor. Works alongside patients and their doctors to improve patient health. Team consists of registered nurses, a pharmacist, an occupational therapist, a physical therapist and a dietitian.

Medical services that are offered include allergy, asthma, and immunology, dermatology, ear, nose, and throat, family medicine, gastroenterology, general surgery, internal medicine, interventional pain, obstetrics and gynecology, ophthalmology, optometry, pediatrics, podiatry, and rheumatology.

Provides assisted living care to those who can no longer remain alone in their home. Services include but are not limited to medication administration, dietary services, laundry and housekeeping services, 24-hour care, daily activities, assistance with activities of daily living, and security options.
