Family Support Coordinators | Black Hills Special Services Cooperative

Provides people and their families with services and supports to meet their unique needs. The Family Support 360 (FS360) Program is self-directed, which means the person and their family choose the services, the staff who provide them, and a schedule based on their needs. FS360 helps people and their family receive state-funded services and other community services, together with natural supports (extended family members, friends, neighbors, church congregations, and community organizations). Family Support Service Coordinators act as an advocate for you and your family. Services and supports include but are not limited to: service coordination, environmental accessibility adaptive equipment, vehicle modifications, companion services, supported employment, personal care, respite care, special medical adaptive equipment and supplies, nutritional supplements, specialized therapies, and recreational and social opportunities.

First Connection: For those who have recently diagnosed with cancer, the Leukemia and Lymphoma Society has provided the opportunity to speak with someone who's been through treatment and learned how to manage the same disease. The Patti Robinson Kaufmann First Connection Program is a peer-to-peer program that matches newly diagnosed patients and their families with trained volunteers who have been touched firsthand by a blood cancer and shared similar experiences. Online Chats: Live, weekly online chats provide a friendly forum to share experiences and chat with others about anything from the initial phase of diagnosis to treatment and survivorship. Each chat is moderated by an oncology social worker and is password-protected. The online chats are not a replacement for psychotherapy or support groups. Out of town travel assistance is not offered to MC residents. Identification and or birth certificate (if needed for the purposes of obtaining employment/housing). The majority of assistance provided is in the form of a no-interest loan for which a lien is created. The client does not receive any bill from County but is expected to repay the debt, starting in a time frame created by the caseworker. Applicants can choose to walk-in to the office or receive a phone call to complete an interview. Those applying for transportation, medications or ID/BC assistance should come into the office to apply.

Guides soldiers and their families to the help they need through a one-stop call center for veterans. The Sea of Goodwill is one place that will have a list of all the various official and unofficial support services available to help veterans. Available 24 hours per day, the Watertown Police Department 911 Center serves as the call center for the Sea of Goodwill.

Auditor and Welfare | Clark County

The Auditor is the keeper of County records and acts as a bookkeeper of County business. The Auditor is responsible for County Commissioner meetings including scheduling, agenda, business follow-up, preparing the County's budget, audit bills or other claims against the County and prepare the warrants in payment of these bills, figuring the mil levy, head election officer of the County, and notary public services available. Some fees may apply Welfare services include medical assistance, utilities assistance for disconnect notices on heating bills, and burial/funeral assistance. All assistance is based on eligibility guidelines. All applicants are required by law to repay the county for assistance given, per SDCL 28-14. County is payer of last resort after all other resources have been utilized.

HIV Positive Services | City of Sioux Falls Health Department

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.
