They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Must have a referral from a medical provider prior to evaluation and treatment depending on insurance requirements. There is also an occupational therapy driver's evaluation and training program available at the West 18th Street location.
