The Extended Care and Rehab Service Line includes the following services: Caregiver support group, spinal cord injury / disorder program, full suite of therapies, chiropractic care, integrative health services, adaptive sports, low vision and visually impaired services, inpatient community living center, pain management, whole health services, community living center, hospice/palliative Care, polytrauma/traumatic brain injury (TBI), geriatrics & extended care (GEC), medical foster home, and home health care.

Provides nursing facility care with 24-hour nursing services in a home-like atmosphere. Services include physical, occupational, and speech therapy, daily activities, nutrition services, hospice services and more. Facility is attached to Bowdle Healthcare Center hospital and clinic for easy access to medical and specialty services. ​

Courage Kenny Rehabilitation Institute (CKRI) is part of Allina Health. Provides a continuum of rehabilitation services for people with short- and long-term conditions, injuries, and disabilities. The goal is to improve health outcomes, make it easier for clients and families to get the right services for their needs, and reduce costs by preventing complications. Programs include: ​Brain Injury Rehabilitation - Helping patients learn new skills to help manage many of the symptoms of brain injury. ​Cancer rehabilitation - Addresses needs at each stage of treatment, helping patients cope with the effects of cancer and its treatment. ​Chronic pain services - Comprehensive outpatient pain management services. ​Comprehensive inpatient neurological rehabilitation - Hospital-based rehabilitation program for those who require close medical monitoring and management of medical conditions. ​Comprehensive stroke rehabilitation program - Evidence-based treatments support individuals who have had a stroke as they regain daily living skills and become as independent as possible. ​Courage Kenny Kids - Offering physical and occupational therapy and speech-language pathology. ​Courage Kenny Spine Centers - Provides specialized, nonsurgical treatment for low back and neck pain. ​Spinal Cord Rehabilitation - Offers inpatient and outpatient services for persons with a spinal cord injury. ​Occupational/physical therapy and speech-language pathology - Providing outpatient rehabilitation therapy for patients of all ages. Home Health can help patients make a good transition from hospital to home, or receive the help and care they need at home. Services include physical and occupational therapy, medication management, wound care, IVs and central lines, personal care, light housekeeping and meal preparation.

Home Health may be considered for people who are recovering from an illness, injury, or surgery, undergoing treatment, people with disabilities, and those who are chronically ill or terminally ill.
