Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Services included personal assistance (i.e. bathing, dressing, eating), companionship, homemaking, skilled nursing care, running errands and provide transportation, respite care, hospital to home care and memory care for people living with Alzheimer's or other form of dementia.

Provides personal cares, homemaking, and transportation services. Personal care services offered (but not limited to) include bathing, dressing, toileting, grooming, and help with mobility. Homemaking services offered include cleaning, dusting, laundry, trash, meal prep, and dishes.

Provides the following home care services: housekeeping (cleaning, laundry, and running errands), companionship (spending time with and supporting individuals in the comfort of their own home), meal preparation (preparing meals, shopping for groceries, and storing leftovers), transportation (to appointments, social events, and family gatherings), basic nursing care, and medication assistance.

Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. Outpatient Services - Individual therapy (for mental health or substance use), Family therapy, Group therapy, Medication management, Psychological assessments and testing, Commitment evaluations, Nursing home consultations, and 24-hour a day crisis intervention via crisis line. Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Offers mental health and substance use services as well as both family and individual case management services which include comprehensive child/adolescent/family services, comprehensive adult/family services, adult/adolescent/family substance abuse services, and outpatient psychiatric medical services. ​Walk-in assessments available during regular business hours and emergency mental health crisis support services available 24 hours a day / 7 days a week. Provides non-medical home care services such as bathing, grooming, dressing, companionship, ambulation assistance, meal prep, feeding assistance, light housekeeping, and more.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Provides a variety of home health service including nursing services (nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.), personal care (bathing, dressing, feeding, ambulation & mobility, incontinence care etc.), homemaking/companion care (laundry, housekeeping, meal preparation, etc.), and structured family caregiving (a program which provides a financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver).

Home Health Services include intermittent skilled nursing and aide services such as bathing and homemaking. ​

​Provides in-home support to individuals with daily activities and personal needs to help individuals maintain their independence and quality of life while remaining in the comfort of their own homes. Services include assistance with activities of daily living (such as bathing, dressing, grooming, toileting, and mobility), light housekeeping, laundry, meal preparation, medication reminders, companionship, social engagement, transportation to help with errands or appointments, and respite care for family caregivers.

Offers a family-centered approach to care and is committed to providing only the most appropriate services based on regular assessments of your needs. Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Provides home health services including personal cares, home making services, respite care, adult companionship, and skilled nursing services.

 Outpatient counseling for children, adolescents, adults, and families. Covering topics such as abuse and/or sexual abuse, anger management, anxiety, attention deficit hyperactivity disorder (ADD/ADHD), depression, emotion and behavior regulation for children, grief and loss, life transition and adjustment issues, marriage and family issues, post traumatic stress (PTSD), relational violence/abuse, self-esteem, stress management, suicide intervention/crisis stabilization, teen problems, wellness counseling, and women's issues. Offers telehealth services. Family Pathways (Family Outreach Services) counselors providing intensive counseling and support services for children in community locations such as home or school. Psychiatric Services include assessments and medication management. CARE - Continuous Assistance, Rehabilitation, and Education services for adults with severe mental illness.

Provides outpatient mental health and substance abuse services, medication management via telehealth services. Home-based children's services available to the community of Chamberlain and the surrounding communities.

Provides in-home care to clients inside their home. They include skilled nursing (e.g. wound care, medication management, physical assessment), homemaking (i.e. light housekeeping, meal preparation, and errands/grocery shopping assistance), personal care, respite care, and companionship.

In-home case management for children, youth, and/or families that are at risk of system involvement (child welfare, juvenile justice, criminal justice, etc.). Assists caregivers strengthen their families and prevent child abuse and neglect through education, skill development and therapeutic crisis management.

Services include cancer services/oncology, cardiology, chaplain services, diabetes care, diagnostic imaging, emergency and trauma care, home health and hospice, home medical equipment, occupational health, orthopedics, pediatrics, rehabilitation services, senior services, sports medicine, telemedicine, wellness services, women's specialty services, and family medicine.

Clients can utilize personalized care to tailor their wants and needs and provide in-home assistance. Services include medication management, toenail care, home assistance, and assistance with activities of daily living (ADL). Also takes part in the South Dakota Department of Human Services Homemaker, Personal Care and Nursing Services.

An in-home rehab program providing a high level of coordination, support, communication, and follow-up with the goal of supporting the client in an independent setting. Services are tailored to the clients needs. This includes accompany clients to their doctor appointments, explaining their insurance explanation of benefits, pharmacist review of medication, and having access to resources to coordinate hospital discharge (i.e. necessary medical equipment, new medications, transportation, and follow-up appointments and services).
