Providing in-home care to those who need assistance with activities of daily living. Services include medical assistance such as in-home memory care, diabetes care, arthritis care, respite care, Alzheimer's and Dementia care, hospice support, chronic illness care, and 24-hour care. They can also assist with personal care (i.e. bathing, dressing), companionship, assistance with daily activities (i.e. meal preparation, light housekeeping), and transportation. These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.

Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Provides in-home services to assist clients to remain in their home including nursing services, personal care services, housekeeping, and companionship.

Provides in-home hospice care including symptom management and emotional/spiritual support in the last 6 months of life.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Provides a variety of home health service including nursing services (nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.), personal care (bathing, dressing, feeding, ambulation & mobility, incontinence care etc.), homemaking/companion care (laundry, housekeeping, meal preparation, etc.), and structured family caregiving (a program which provides a financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver).

Hospice services use a team approach of physicians, nurses, social workers, home health aides, volunteers, chaplains, family members and others. Hospice strives to provide quality medical care, pain and symptom management, and emotional and spiritual support tailored to help ensure a higher quality of life. Nursing visits are made on a regular schedule and an on-call 24-hour basis as needed. Additional services include inpatient and physician-directed services, interdisciplinary, integrated services, and bereavement counseling.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.
