Home Health Services include intermittent skilled nursing and aide services such as bathing and homemaking. ​

Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.

Residents experience meaningful daily routines, sensory-rich activities, and personalized care that promote comfort, dignity, and connection. Services include light housekeeping, laundry service, salon services, vital signs/weight monitoring, and healthcare coordination.

Supporting caregivers that help their Medicaid-eligible loved ones at home by providing an expert care team, emotional support, and financial assistance.

Provides in-home care including personal cares (such as bathing, grooming, dressing, transfers, and ambulation) and companionship services (such as meal prep, transportation, grocery shopping, laundry, and light housekeeping).

Services include skilled nursing, physical therapy, occupational therapy, speech therapy, personal care, dressing, medication assistance, lab draws, and home infusion therapy. Also specializes in providing home care after surgery.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Provides friendship and free in-home assistance with activities to help older persons remain living independently in their own homes. Major focus is clients over 60. Small percentage of clients served are adults (over 21) with disabilities. Eligibility is based on need for service. Senior Companions must be age 55 or older, meet an income guideline, and be willing to serve a minimum of 15 hours a week and to reach out and provide service to others. Senior Companions are paid a tax-free stipend for their volunteer work and mileage.

Provides independent living and assisted living services. Independent living services include nutrition services, weekly housekeeping, life enrichment programming, and 24-hour on site staffing. Assisted living offers a more supportive environment offering 24-hour personal care services including nutrition services, housekeeping, life enrichment programming, and personal assistance with activities of daily living. Also provides memory care services.

Helping connect those who provide care and support for another adult with the AARP Caregiver Resource Center.

Services include in-home care, companionship, daily activities, housekeeping, laundry, meal preparation, medication management, dementia management, mobility assistance, personal care, respite care, shopping/errand assistance, and structured family caregiving.

Provides long-term nursing facility care, short-term rehabilitation, and outpatient rehabilitation services in a home-like environment with 24-7 care. Independent living services provides a secure environment with dining services, housekeeping services, emergency call buttons in each apartment, social services, and more.

Providing health care and supportive services in a patient's home. Home Health may be considered for people who are recovering from an illness, injury, or surgery, undergoing treatment, people with disabilities, and those who are chronically ill or terminally ill. Services offered are skilled nursing care, home health aides for personal care, rehabilitation therapy (physical therapy, occupational therapy, and speech therapy), social services, patient/family education, medication monitoring, and nutrition consultation.

Provides assisted living care to those who can no longer remain alone in their home. Services include but are not limited to medication administration, dietary services, laundry and housekeeping services, 24-hour care, daily activities, assistance with activities of daily living, and security options.

Services included personal assistance (i.e. bathing, dressing, eating), companionship, homemaking, skilled nursing care, running errands and provide transportation, respite care, hospital to home care and memory care for people living with Alzheimer's or other form of dementia.

Offers senior living services with an individualized and adaptable care plan that addresses physical, social, emotional, and spiritual needs. Services and amenities include private rooms and bathrooms, dining and common areas, outdoor courtyard, life enrichment activities, 24/7 nursing care, medication management, nutrition services, daily housekeeping and laundry, assisted bathing, behavior modification and management, incontinence care, and fall prevention management.

Provides in-home personal care services including basic needs assistance (eating or feeding, companionship, and medication reminders), personal hygiene assistance (grooming, dressing, bathing, and mobility), and homemaking services (indoor and outdoor home maintenance, meal prep, and shopping). Also provides structured family caregiving services.

Provides assisted living care including the following services: 24-hour staffing, dietary services, medication administration, recreational activities, laundry and housekeeping, emergency call systems, assistance with activities of daily living, and coordination of care needs.

Allows family members to get paid to care for an aging adult or an adult with a disability in the person's on home or in the caregiver's home The goal is to enable the person to live in their community while getting needed care and supervision from a paid family caregiver. The caregiver receivers ongoing support and training from a provider.

Provides quality home health care to residents of the Sioux Empire, Vermillion, and Chamberlain communities. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Provides individualized care for elderly individuals with dementia or other cognitive impairment as well as provides support for their caregivers.

Long Term Services and Supports provides opportunities to enhance the quality of life for older adults, adults with disabilities, and their caregivers through home and community based services. Services Include: home care services to help adults remain in their home, safety and help moving around, nutrition and meals, Senior Health Information and Insurance Education (SHIINE) providing individuals information about benefits they can receive through Medicare, caregiver support, Structured Family Caregiving, residential living services, Ombudsman program providing advocacy and education for residents, family, and staff in long term care settings, and adult protective services providing interventions to protect vulnerable adults from abuse, neglect, and exploitation.

Provides homemaking, personal care, respite, and companion services. Homemaking services include housekeeping, meal prep, meal planning, cooking, laundry, and other household tasks. Personal care services include assistance with showering, bathing, dressing, hair, and grooming. Respite services provide a break to a primary caregiver. and Companion services offer companionship for recreational, social, or home activities.
