Provides an individualized and adaptable care plan that addresses physical, social, emotional, and spiritual needs. Services and amenities include private rooms and bathrooms, dining and common areas, outdoor courtyard, life enrichment activities, 24/7 nursing care, medication management, nutrition services, daily housekeeping and laundry, assisted bathing, behavior modification and management, incontinence care, and fall prevention management.

Provides friendship and free in-home assistance with activities to help older persons remain living independently in their own homes. Major focus is clients over 60. Small percentage of clients served are adults (over 21) with disabilities. Eligibility is based on need for service. Senior Companions must be age 55 or older, meet an income guideline, and be willing to serve a minimum of 15 hours a week and to reach out and provide service to others. Senior Companions are paid a tax-free stipend for their volunteer work and mileage.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Examples of services include meals, home care, transportation, caregiver training and education, or respite care. Eldercare Locator is administered by USAging.

Offers three levels of high acuity care specializing in Memory Healthcare services. Residents experience meaningful daily routines, sensory-rich activities, and personalized care that promote comfort, dignity, and connection. Services include light housekeeping, laundry service, salon services, vital signs/weight monitoring, and healthcare coordination.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Provides individualized in-home care and in home-rehab allowing people to remain in their home while receiving many of the professional health services available at a higher level of care.
