The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Small percentage of clients served are adults (over 21) with disabilities. Eligibility is based on need for service. Senior Companions must be age 55 or older, meet an income guideline, and be willing to serve a minimum of 15 hours a week and to reach out and provide service to others. Senior Companions are paid a tax-free stipend for their volunteer work and mileage.

Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.

Independent Cottages are private one-bedroom apartments created for seniors seeking maintenance-free living with optional support. Residences are rented on a monthly basis and include one meal per day, plus housekeeping and laundry services every other week. Customized service packages are available to provide additional assistance as needed, such as enhanced housekeeping or laundry, medication management, and basic personal care support.

Services include skilled nursing, physical therapy, occupational therapy, speech therapy, personal care, dressing, medication assistance, lab draws, and home infusion therapy. Also specializes in providing home care after surgery.

Personal care services include assistance with showering, bathing, dressing, hair, and grooming. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.
