The CHWs have lived experience as immigrants and refugees, so have a unique ability to connect with and understand barriers.

Walk-in assessments are available at the Watertown and Sisseton locations. Also offers and telehealth services.

One Heart Clinic: Providing primary family care to patients with a focus on patients with a primary language of Spanish. Pharmacy: Convenient, on-site location, CHCBH patients are able to fill most of their medications the same day as appointment.

Provides peer support and financial assistance to individuals and families who have lost a loved one to suicide. Examples consist of financial assistance for grief counseling, burial expenses, clean-up costs, legal fees, and other suicide-related expenses. The Program Coordinator supports clients with referrals and resources to additional financial assistance programs and/or therapeutic support if needed. Also provides peer and emotional support and resource referrals for a year. Peer support is provided through telehealth meetings and check-ins.

Community Health Worker (CHW) services are provided through Health Connect of South Dakota. This includes connecting community members to information or services on housing, transportation, food access, employment, financial literacy and more. Case management services provided include health system navigation and resource coordination (i.e. establishing prenatal healthcare coverage and scheduling appointments), Health promotion and coaching (i.e. prenatal and nutrition education, budgeting, application assistance for programs), Housing search assistance, Transitional housing, and Transportation coordination to/from appointments.

Provides direct patient health care services including: promoting education and awareness of proper diet, exercise and health care practices; providing health care to patients with diabetes complications; education on reducing/preventing disease and accidents; providing transportation to health care facilities for patients; assisting IHS in providing baby and well-child immunizations checks; informing proper agencies of any violations and conducting follow-ups; gerontology for the elderly population; reducing rate of teenage pregnancy, STD's, with awareness and prevention practices; conducting Wellness, Health and Nutrition Awareness workshops/clinics; providing direct health care services to physically impaired individuals; providing medical transportation by means of the Transit Program; providing community health worker services; and providing animal care services to canine and feline species.

Community Health Education / promotion / wellness and disease / accident prevention. ​Provides transportation for medical purposes for individuals who qualify, transit tickets for medical appointments for able bodied individuals and Dial-A-Ride tickets, Community Health Worker services for high risk clients, and medication delivery for qualified individuals.
