Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Community Health Worker services providing case management and peer to peer support to face immediate challenges and discover holistic solutions to achieve balanced lives.

Community Health Education / promotion / wellness and disease / accident prevention. ​Provides transportation for medical purposes for individuals who qualify, transit tickets for medical appointments for able bodied individuals and Dial-A-Ride tickets, Community Health Worker services for high risk clients, and medication delivery for qualified individuals. There are 3 CHR Specialists and 1 CHR Generalist that help with medical transportation and case management for high-risk populations.

Provides direct patient health care services including: promoting education and awareness of proper diet, exercise and health care practices; providing health care to patients with diabetes complications; education on reducing/preventing disease and accidents; providing transportation to health care facilities for patients; assisting IHS in providing baby and well-child immunizations checks; informing proper agencies of any violations and conducting follow-ups; gerontology for the elderly population; reducing rate of teenage pregnancy, STD's, with awareness and prevention practices; conducting Wellness, Health and Nutrition Awareness workshops/clinics; providing direct health care services to physically impaired individuals; providing medical transportation by means of the Transit Program; providing community health worker services; and providing animal care services to canine and feline species.

Community mental health center offering the following services: Moral Recognition Therapy (MRT) Group for adolescents and adults, Dialectical Behavioral Therapy (DBT), individual counseling, marriage and family counseling, psychotropic medication management, domestic violence evaluations and follow up services, psychiatric evaluations and medication management. Walk-in assessments are available at the Watertown and Sisseton locations. Also offers and telehealth services.

The Community Health Workers (CHWs) at the Avera Community Health Resource Center help others connect to and navigate healthcare, apply for jobs at Avera, and assist in connecting with other available resources within the community. The CHWs have lived experience as immigrants and refugees, so have a unique ability to connect with and understand barriers.

Outpatient, voluntary walk-in center for youth in middle and high school who are in crisis or considering suicide. Services include Walk-in Urgent Visits (for both medical and dental), Schedule and same day appointments, Family Practice and Internal Medicine Primary Care for adults, Dental Care for all ages (including teeth cleanings), Sexual and Reproductive Health Services, On-site lab and X-Rays, On-Site Pharmacy, and After-Hours On-Call Provider 365 days a year. Primary and Pediatric Medical Clinic: Most routine medical services and several specialized fields including: physicals, immunizations, preventative medicine, chronic disease management, and counseling. Well child checkups, vaccinations, general pediatric health care. Mental Health: Offers outpatient counseling and psychiatric services, to both children and adults. Case management is offered to adults and families. Dental Clinic: Provides emergency, preventive, and oral health care. Accepts Medicaid, Private Insurance, Medicare, free of charge for the homeless and on a sliding fee scale for uninsured. One Heart Clinic: Providing primary family care to patients with a focus on patients with a primary language of Spanish. Pharmacy: Convenient, on-site location, CHCBH patients are able to fill most of their medications the same day as appointment.

Case management services provided include health system navigation and resource coordination (i.e. establishing prenatal healthcare coverage and scheduling appointments), Health promotion and coaching (i.e. prenatal and nutrition education, budgeting, application assistance for programs), Housing search assistance, Transitional housing, and Transportation coordination to/from appointments.

Community Health Worker (CHW) services are provided through Health Connect of South Dakota. This includes connecting community members to information or services on housing, transportation, food access, employment, financial literacy and more. Provides peer support and financial assistance to individuals and families who have lost a loved one to suicide. Examples consist of financial assistance for grief counseling, burial expenses, clean-up costs, legal fees, and other suicide-related expenses. The Program Coordinator supports clients with referrals and resources to additional financial assistance programs and/or therapeutic support if needed. Also provides peer and emotional support and resource referrals for a year. Peer support is provided through telehealth meetings and check-ins.

For anyone facing an unplanned pregnancy, All About U provides crisis pregnancy counseling, help finding emotional support, help finding necessary resources, emotion support throughout adoption and beyond, if adoption is chosen, emergency transitional housing available, and general case management for resources: connection to prenatal care, housing, food, clothing, addiction, and more. For adoptive families, All About U provides training to help them through the adoption process and prepare them to be an adoptive family, home studies, and on-going support for adoptive families.
