Provides the following services - Advocacy: Teaches people with disabilities the skills needed to advocate for themselves. Independent Living Skills: Provides hands-on training in daily-living skills including cooking, budgeting, organizing, and more. Information and referral: Connects people with resources which can provide needed information to assist individuals to live independently. Peer counseling: Provides opportunities for people with disabilities to engage with others in small or large groups or on an individual basis. Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria.
