Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

A zoom-based monthly meeting that welcomes survivors of brain injury (traumatic injury, acquired injury, neurological illness). The focus of the meeting is to allow all members to socialize with others, share resources, and offer support to one another. This group is facilitated by a speech pathologist but no formal therapy services are conducted.

Any individual who has had a brain injury is welcome to attend. The support group is also available for family members. Brain injuries can be obtained from accidents, falls, multiple concussions, tumors, stokes, lack of oxygen, epilepsy and Parkinson's, to name a few. The brain injury may be a recent injury or one that has been acquired for many years.

The Extended Care and Rehab Service Line includes the following services: Caregiver support group, spinal cord injury / disorder program, full suite of therapies, chiropractic care, integrative health services, adaptive sports, low vision and visually impaired services, inpatient community living center, pain management, whole health services, community living center, hospice/palliative Care, polytrauma/traumatic brain injury (TBI), geriatrics & extended care (GEC), medical foster home, and home health care.

​ A support group for anyone who has sustained a brain injury. Families and friends are encouraged to attend. At times the group will divide into a group for family/friends and a group for individuals who have sustained a brain injury. Call to be added to the mailing list. This group is co-sponsored by Avera McKennan and Sanford Hospitals.

Also provides a balance and dizziness clinic and a Spine Navigator who offers information on services provided, treatable conditions, and how to start the referral process. The spine navigator helps patients and family members understand aspects of diagnosis, treatment, and recover.

The main focus of the group is to provide opportunity to socialize with others, provide resources, and offer support. This group is facilitated by a speech pathologist, however, no therapy is performed during meetings.
