Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Provides home care and nursing services to anyone 18 years or older who needs assistance with activities of daily living (ADLs) due to illness, injury, disability, or aging. Services take into consideration spiritual, physical and emotional needs. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Provides a variety of home health service including nursing services (nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.), personal care (bathing, dressing, feeding, ambulation & mobility, incontinence care etc.), homemaking/companion care (laundry, housekeeping, meal preparation, etc.), and structured family caregiving (a program which provides a financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver).

Long Term Services and Supports provides opportunities to enhance the quality of life for older adults, adults with disabilities, and their caregivers through home and community based services. Services Include: home care services to help adults remain in their home, safety and help moving around, nutrition and meals, Senior Health Information and Insurance Education (SHIINE) providing individuals information about benefits they can receive through Medicare, caregiver support, Structured Family Caregiving, residential living services, Ombudsman program providing advocacy and education for residents, family, and staff in long term care settings, and adult protective services providing interventions to protect vulnerable adults from abuse, neglect, and exploitation.

Provides medical care to maximize comfort and quality of life for patients facing terminal illnesses. It can take place anywhere a patient might reside such as in the home, a nursing home, or in a short-term in-patient hospital setting. Services include medical social services, spiritual care, personal care services, home management services, physical, occupational, and speech therapy services, nutritional services, medication management, medical equipment and supplies, short-term-in-patient care, and bereavement/aftercare services for at least a year.

Provides medical and non-medical home care services including assistance with personal care, homemaking, companionship, respite care, vital sign monitoring, nail care, catheter care, and medication reminders.

Huron Regional Medical Center Home Health uses a combined team approach including physicians, registered nurses, nurse aides, physical, occupational and speech therapists, and social workers. Services provided may include medication monitoring and organization, education on recently diagnosed illness or medical condition, wound care, catheter care and irrigation, pain management through medication or comfort methods, physical, speech and occupational therapy, communication and daily living tasks, and assistance with personal care.

Providing in-home care to those who need assistance with activities of daily living. Services include medical assistance such as in-home memory care, diabetes care, arthritis care, respite care, Alzheimer's and Dementia care, hospice support, chronic illness care, 24-hour care, and nurse directed care (i.e. medication administration, wound care). They can also assist with personal care (i.e. bathing, dressing), companionship, assistance with daily activities (i.e. meal preparation, light housekeeping), and transportation. These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.

Visiting Nurse Service provides skilled care in the home. Works closely with clients, family, and physicians to develop a care plan that meets the particular needs of each individual.

Provides in-home services including medical care (wound care, medication administration, medical tests, skilled nursing services, and hospice care), companion care (escort/transportation, meal preparation, protective supervision, respite care, shopping assistance), personal cares (bathing, dressing and grooming, body care, assistance with doctor's appointments, feeding assistance, positioning, standby assistance, total assistance, and ambulation assistance), and homemaker services (cleaning assistance and other light housekeeping).

Home Health can help patients make a good transition from hospital to home, or receive the help and care they need at home. Services include physical and occupational therapy, medication management, wound care, IVs and central lines, personal care, light housekeeping and meal preparation.
