The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. Allows family members to get paid to care for an aging adult or an adult with a disability in the person's on home or in the caregiver's home The goal is to enable the person to live in their community while getting needed care and supervision from a paid family caregiver. Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.
