Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Services included personal assistance (i.e. bathing, dressing, eating), companionship, homemaking, skilled nursing care, running errands and provide transportation, respite care, hospital to home care and memory care for people living with Alzheimer's or other form of dementia.

Provides personal cares, homemaking, and transportation services. Personal care services offered (but not limited to) include bathing, dressing, toileting, grooming, and help with mobility. Homemaking services offered include cleaning, dusting, laundry, trash, meal prep, and dishes.

Provides the following home care services: housekeeping (cleaning, laundry, and running errands), companionship (spending time with and supporting individuals in the comfort of their own home), meal preparation (preparing meals, shopping for groceries, and storing leftovers), transportation (to appointments, social events, and family gatherings), basic nursing care, and medication assistance.

Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. Provides non-medical home care services such as bathing, grooming, dressing, companionship, ambulation assistance, meal prep, feeding assistance, light housekeeping, and more.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Provides a variety of home health service including nursing services (nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.), personal care (bathing, dressing, feeding, ambulation & mobility, incontinence care etc.), homemaking/companion care (laundry, housekeeping, meal preparation, etc.), and structured family caregiving (a program which provides a financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver).

Home Health Services include intermittent skilled nursing and aide services such as bathing and homemaking. ​

Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Provides home health services including personal cares, home making services, respite care, adult companionship, and skilled nursing services.

Provides in-home care to clients inside their home. They include skilled nursing (e.g. wound care, medication management, physical assessment), homemaking (i.e. light housekeeping, meal preparation, and errands/grocery shopping assistance), personal care, respite care, and companionship.

Services include cancer services/oncology, cardiology, chaplain services, diabetes care, diagnostic imaging, emergency and trauma care, home health and hospice, home medical equipment, occupational health, orthopedics, pediatrics, rehabilitation services, senior services, sports medicine, telemedicine, wellness services, women's specialty services, and family medicine.

Clients can utilize personalized care to tailor their wants and needs and provide in-home assistance. Services include medication management, toenail care, home assistance, and assistance with activities of daily living (ADL). Also takes part in the South Dakota Department of Human Services Homemaker, Personal Care and Nursing Services.

Services provided include personal care and daily living assistance, light housekeeping, meal preparation, transportation, doctor appointment accompaniment, Dementia/Alzheimer's care, respite care, pet care, and companion and homemaker services.

Providing a comprehensive range of services for clients to stay in their homes as well as preserve dignity and independence. Services include respite and companion care, home support, personal care, and transportation. Specialize in working with people with Dementia and Alzheimer's Disease.

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Provides in-home personal care services including basic needs assistance (eating or feeding, companionship, and medication reminders), personal hygiene assistance (grooming, dressing, bathing, and mobility), and homemaking services (indoor and outdoor home maintenance, meal prep, and shopping). Also provides structured family caregiving services.

Offers skilled nursing services, homemaking, personal care, bathing assistance, and shopping assistance.

Providing inpatient-level care for individuals who can safely receive care in the home. This consists of visits with a Registered Nurse, Advanced Practice Provider and Physician visiting in person or virtually.
