Providing health care and supportive services in a patient's home. Home Health may be considered for people who are recovering from an illness, injury, or surgery, undergoing treatment, people with disabilities, and those who are chronically ill or terminally ill. Services offered are skilled nursing care, home health aides for personal care, rehabilitation therapy (physical therapy, occupational therapy, and speech therapy), social services, patient/family education, medication monitoring, and nutrition consultation.

Services include skilled nursing, physical therapy, occupational therapy, speech therapy, personal care, dressing, medication assistance, lab draws, and home infusion therapy. Also specializes in providing home care after surgery.

Offers personalized care plans tailored to individual needs trough 24/7 support, flexible services, and a family-focused approach. Provides companionship services such as emotional support and social interaction through conversations, participating in hobbies, accompanying clients to appointments, and ensuring a comforting presence to reduce feelings of loneliness and isolation. Also provides personal care assistance including assistance with activities of daily living (bathing, grooming, dressing, toileting, mobility support, medication reminders) to support clients independence and well-being. Medication Management assists in ensuring timely and accurate medication administration which aims to promote clients health a safety by preventing missed doses or potential medication errors.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Services provided are companionship, personal care, light housekeeping, respite care, and nursing services. Also offers a free in-home evaluation for clients and their families to determine their goals and needs to achieve independence.

Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. Services include assistance with activities of daily living (such as bathing, dressing, grooming, toileting, and mobility), light housekeeping, laundry, meal preparation, medication reminders, companionship, social engagement, transportation to help with errands or appointments, and respite care for family caregivers.

Provides the following home care services: housekeeping (cleaning, laundry, and running errands), companionship (spending time with and supporting individuals in the comfort of their own home), meal preparation (preparing meals, shopping for groceries, and storing leftovers), transportation (to appointments, social events, and family gatherings), basic nursing care, and medication assistance.

Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Services include 24-hour room service menu for patients, aromatherapy, grief/bereavement counseling, music enrichment, pain management, pet therapy, spiritual and social support, and remembrance items and activities for the family. Provides temporary relief services for family members or primary caregivers, allowing them time away from caregiving duties while ensuring their loved one continues to receive safe and supportive care in the home environment.

Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.
