Provides quality home health care to residents of the Sioux Empire, Vermillion, and Chamberlain communities. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Helping older adults remain in their homes and be independent for as long as possible. Services offered are assisting individuals with their daily chores (i.e. personal care, light housekeeping, and laundry).

An in-home rehab program providing a high level of coordination, support, communication, and follow-up with the goal of supporting the client in an independent setting. Services are tailored to the clients needs. This includes accompany clients to their doctor appointments, explaining their insurance explanation of benefits, pharmacist review of medication, and having access to resources to coordinate hospital discharge (i.e. necessary medical equipment, new medications, transportation, and follow-up appointments and services).

Services include skilled nursing, physical therapy, occupational therapy, speech therapy, personal care, dressing, medication assistance, lab draws, and home infusion therapy. Also specializes in providing home care after surgery.

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Services are provided within the patient's home. The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living.
