Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Offers a family-centered approach to care and is committed to providing only the most appropriate services based on regular assessments of your needs. Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Services include assistance with activities of daily living (such as bathing, dressing, grooming, toileting, and mobility), light housekeeping, laundry, meal preparation, medication reminders, companionship, social engagement, transportation to help with errands or appointments, and respite care for family caregivers.

Provides a number of personalized care services for elderly and disabled individuals including personal care, homemaking, nursing care, and respite care.

Allows family members to get paid to care for an aging adult or an adult with a disability in the person's on home or in the caregiver's home The goal is to enable the person to live in their community while getting needed care and supervision from a paid family caregiver.
