Visiting Nurse Service provides skilled care in the home. Work closely with clients, family, and physicians to develop a care plan that meets the particular needs of each individual.

Providing inpatient-level care for individuals who can safely receive care in the home. This consists of visits with a Registered Nurse, Advanced Practice Provider and Physician visiting in person or virtually.

Provides personal attendant services for in-home assistance with activities of daily living, housekeeping, companionship, bathing, dressing, and eating.

Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Offers compassionate care, services, equipment, and medications to support quality at-home care. A range of comprehensive services are available including the following: home health, palliative care, home medical equipment, home infusion, hospice care, retail pharmacy, specialty pharmacy, and home modifications.

Offered skilled home health services including nurses, physical therapy, occupational therapy, speech therapy, and home health aides providing care, support and education to allow patients to manage health conditions at home.

Home Health services includes the following: Personal Care: Personal Hygiene and Grooming, Meal Prep, feeding, and clean-up, medication reminders, transportation to and from appointments, errands, and events, exercise and nutrition support, and safety and fall risk appraisals. Companion Care: Doing day-to-day activities, laundry and light housekeeping, 24-hour care, and walks outside/inside. Respite Care is available for families to relieve them of their duties while on vacation or out for a few hours. Also offers supplemental care if loved one resides in an assisted living or similar medical facility.

Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Huron Regional Medical Center Home Health uses a combined team approach including physicians, registered nurses, nurse aides, physical, occupational and speech therapists, and social workers. Services provided may include medication monitoring and organization, education on recently diagnosed illness or medical condition, wound care, catheter care and irrigation, pain management through medication or comfort methods, physical, speech and occupational therapy, communication and daily living tasks, and assistance with personal care.

Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Providing in-home care and nursing care to to those who need assistance with both non-skilled and skilled services to help with activities of daily living Skilled Services are provided by licensed professionals, including medical assistance, therapies, and other health-related services tailored to individual needs. These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.

Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.
