Provides home health and hospice services to individuals and their loved ones. The Home Health program delivers skilled nursing services in the home, physical therapy, occupational therapy, speech therapy, and personal care of homemaking services. Hospice Care includes routine home care, continuous care, general inpatient, and respite care.

Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental health Services: individual, couples, and group psychotherapy Telemental health available Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. The following programs and specialty care are available by telehealth: Cardiology, Diabetes Management, Endocrinology, Gastroenterology, Genomic Medicine, Infectious Disease, Nephrology, Neurology, Nutrition, Oncology, Orthopedics, Pain Management, Physical Therapy, Prosthetics, Pulmonary, and VIST (low Vision).

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Offers personalized care plans tailored to individual needs trough 24/7 support, flexible services, and a family-focused approach. Provides companionship services such as emotional support and social interaction through conversations, participating in hobbies, accompanying clients to appointments, and ensuring a comforting presence to reduce feelings of loneliness and isolation. Also provides personal care assistance including assistance with activities of daily living (bathing, grooming, dressing, toileting, mobility support, medication reminders) to support clients independence and well-being. Medication management assists in ensuring timely and accurate medication administration which aims to promote clients health a safety by preventing missed doses or potential medication errors.

Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities.
