Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.
