The Monument Health pain management team will work with you and your family to create a plan, monitor your progress, and develop the skills to regain control of your life. The team creates an individualized plan for every patient. They employ an approach that combines medication management and interventional therapy to help you regain a higher level of function. Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Provides home care and nursing services to anyone 18 years or older who needs assistance with activities of daily living (ADLs) due to illness, injury, disability, or aging. Services take into consideration spiritual, physical and emotional needs. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

Whether an adult, teen or child struggling with depression, anxiety, relationship issues, family conflict, behavioral challenges, trauma, pain, loss or any area of difficulty, staff are available to help and offer telehealth services.
