Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. Client groups include DBT (Dialectical Behavior Therapy) and MRT (Moral Reconation Therapy).

Clinic provides primary care, mental health, and telehealth services. No emergency services are provided at the clinic. Specialty services include: Women's Health Care, Whole Care, Military Sexual Trauma Care, PTSD Care, Returning Service Member Care Management. Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

A group mental health counseling practice serving children and their families age 3 - adult. Provides school-based counseling as well as in-office appointments and telemental health.

• Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week.

Hospitalization Process: If hospitalization is required, patients are provided a full physical examination and medical evaluation at the Rapid City Monument Hospital Emergency Department prior to hospitalization. Please proceed to the Emergency Department first or if emergent treatment is needed, call local law enforcement or 911. ​ Sponsored by The University of South Dakota Sanford School of Medicine medical students under the supervision of internal medicine physicians, provide health care with the goal is to provide health care to uninsured patients over the age of 18 while providing a learning opportunity for medical students. Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.
