Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

Provides Buprenorphine Based MAT (Medication assisted treatment) to help people with an opioid use disorder abstain from or reduce their use of opioids. Can only serve people within a 50 mile radius of Aberdeen.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Provide medication management and supportive therapy to children (ages 10 and up), adolescents, and adults. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides inpatient mental health care to adolescents and adults who are experiencing acute psychiatric distress and require assessment and short term inpatient intervention with services including psychiatric evaluation and stabilization, therapy services, spiritual services, dietary consultation, and Chemical Dependency Consultation.

CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders.
