Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provide medication management and supportive therapy to children (ages 10 and up), adolescents, and adults. Offers counseling the following: individual counseling, adolescent psychiatry, medication management, LGBTQ counseling, mental health screening, adult psychiatry, adolescent/youth counseling, gender identity counseling, parent counseling and telemental health services.

Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. The following programs and specialty care are available by telehealth: Cardiology, Diabetes Management, Endocrinology, Gastroenterology, Genomic Medicine, Infectious Disease, Nephrology, Neurology, Nutrition, Oncology, Orthopedics, Pain Management, Physical Therapy, Prosthetics, Pulmonary, and VIST (low Vision).

Provides inpatient mental health care to adolescents and adults who are experiencing acute psychiatric distress and require assessment and short term inpatient intervention with services including psychiatric evaluation and stabilization, therapy services, spiritual services, dietary consultation, and Chemical Dependency Consultation.

Provides counseling services to adolescents and adults struggling with anxiety, ADD/ADHD, depression, panic, bipolar disorders, and post-traumatic stress disorder (PTSD) in order to bridge the gap while patients are finding long-term counselors. Offers services in-person in Aberdeen, Sioux Falls, and Yankton plus serving statewide via telehealth.

Psychiatric care for people with anxiety, depression, mental illness, and addiction disorders. Staffed with a psychiatrist and counselor. Provides Buprenorphine Based MAT (Medication assisted treatment) to help people with an opioid use disorder abstain from or reduce their use of opioids. Can only serve people within a 50 mile radius of Aberdeen.

Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Clients receive help with depression, anxiety, behavioral problems, adjustment issues, grief and loss, as well as other mental health issues. An adult outpatient, time-limited, intensive, and coordinated clinical treatment program with the goal of stabilization with the intention of averting inpatient hospitalization or reducing the length of a hospital stay. ​Partial Hospital Program (PHP): Serves as a positive alternative when individual counseling does not fully meet the needs of the individual and when inpatient care is not necessary. 5 days per week, 4 hours per day. Intensive Outpatient Program (IOP): The goal of program is to provide support while promoting independence, growth, individuality and self-reliance. Clients are assisted in learning new and more effective coping skills while remaining in their homes and their jobs. Generally 7 - 11 hours of structured programming per week consisting of counseling and education groups.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. ​ Sponsored by The University of South Dakota Sanford School of Medicine medical students under the supervision of internal medicine physicians, provide health care with the goal is to provide health care to uninsured patients over the age of 18 while providing a learning opportunity for medical students. Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.
