Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Culturally sensitive mental health care for adults and adolescents. Focuses on medication management, mental health screenings for a variety of mental health issues, spravato (eskatamine) for treatment resistant depression, and photobiomodulation therapy (light bed treatment.

Approaching mental health treatment with a holistic approach. Utilizes a combination of medications, supplements, psychotherapy, and lifestyle modifications to improve mental health. Also provides an individualized treatment plan and offers referrals to therapists, counselors, dietitians, health coaches, and wellness professionals. This clinic offers esketamine (Spravato) for treatment-resistant depression.

Individual, group, couples, and family counseling for all ages. Children's services include case management and a home-based program that provides a wide range of therapeutic support. Early childhood mental health services are also available including play therapy. CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders. Components of the program include residential services, independent living training, educational services, and therapeutic and psychiatric services (individual and group therapy).

Cedar Village Assisted Living Facility is a therapeutic place designed to provide permanent supported housing for individuals who are homeless because of severe and persistent mental illness.
