​ Sponsored by The University of South Dakota Sanford School of Medicine medical students under the supervision of internal medicine physicians, provide health care with the goal is to provide health care to uninsured patients over the age of 18 while providing a learning opportunity for medical students. Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.

An adult outpatient, time-limited, intensive, and coordinated clinical treatment program with the goal of stabilization with the intention of averting inpatient hospitalization or reducing the length of a hospital stay. ​Partial Hospital Program (PHP): Serves as a positive alternative when individual counseling does not fully meet the needs of the individual and when inpatient care is not necessary. 5 days per week, 4 hours per day. Intensive Outpatient Program (IOP): The goal of program is to provide support while promoting independence, growth, individuality and self-reliance. Clients are assisted in learning new and more effective coping skills while remaining in their homes and their jobs. Generally 7 - 11 hours of structured programming per week consisting of counseling and education groups.

Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. A psychiatric hospital offering a wide range of services for geriatric, adult, adolescent, and child patients. This center approaches patient care with a special emphasis on trauma-informed care as well as biological, psychological, and social concerns. - Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week. Inpatient Services: Specialized services include group and individual therapy, diagnostic testing, psychological evaluations, and family therapy. Recreational therapists provide a healing and therapeutic environment. Chemical Dependency Specialists perform detailed evaluations and treatment planning for patients struggling with addiction. Cedar Village Assisted Living Facility is a therapeutic place designed to provide permanent supported housing for individuals who are homeless because of severe and persistent mental illness.

Provides medication management and psychotherapy for ages 5 and up. Providers consist of a psychiatrist and 2 nurse practitioners. In-person appointments, as well as telehealth services available. Spravato treatments offered at the clinic site. Transcranial magnetic stimulation (TMS) also available. Mental health services include, but are not limited to the following: depression; anxiety; panic; insomnia; alcohol abuse; personality disorders; opioid abuse; binge eating; etc.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.
