Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

A psychiatric hospital offering a wide range of services for geriatric, adult, adolescent, and child patients. This center approaches patient care with a special emphasis on trauma-informed care as well as biological, psychological, and social concerns. - Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week. Inpatient Services: Specialized services include group and individual therapy, diagnostic testing, psychological evaluations, and family therapy. Recreational therapists provide a healing and therapeutic environment. Chemical Dependency Specialists perform detailed evaluations and treatment planning for patients struggling with addiction. Intensive case management is available to coordinate external services for the hospitalized patient. The hospital's clergy is available upon patient request or a patient's own spiritual leader is welcome to provide comfort. Hospitalization Process: If hospitalization is required, patients are provided a full physical examination and medical evaluation at the Rapid City Monument Hospital Emergency Department prior to hospitalization. Please proceed to the Emergency Department first or if emergent treatment is needed, call local law enforcement or 911. Outpatient Clinic: Offers individual and family counseling services for patients of all ages as well as psychiatric diagnostic interviewing and medication management for patients of all ages.

Offering health care services such as primary care, laboratory and pathology, mental health care, treatment in post traumatic stress disorder (PTSD), and suicide prevention.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Outpatient mental health services for older adults experiencing mental health challenges associated with aging. Patients develop coping and communication skills they need to live a happier and healthier life. Services offered include a confidential, comprehensive assessment, individual therapy, group therapy, family therapy, medication education and management, and after-care planning. Also set aside time individual psychotherapy and check each participant's vitals. Telehealth is available for patients to participant via the internet.

Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

A treatment team, headed by a psychiatrist, works to evaluate current symptoms and develop a positive treatment plan to address concerns. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Children's services include case management and a home-based program that provides a wide range of therapeutic support. Early childhood mental health services are also available including play therapy. Areas of specialization include depression, anxiety, grief, parenting challenges, attention deficit hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), stress management, and issues related to trauma or abuse.
