Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Provides traditional outpatient mental health therapy for individuals, families, children, couples, and groups. Also provides psychiatric evaluations, medication management, and psychological assessments. Services can be accessed both in person and via telehealth services. Children's services include case management and a home-based program that provides a wide range of therapeutic support. Early childhood mental health services are also available including play therapy. Areas of specialization include depression, anxiety, grief, parenting challenges, attention deficit hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), stress management, and issues related to trauma or abuse.

Outpatient mental health services for older adults experiencing mental health challenges associated with aging. Patients develop coping and communication skills they need to live a happier and healthier life. Services offered include a confidential, comprehensive assessment, individual therapy, group therapy, family therapy, medication education and management, and after-care planning. Also set aside time individual psychotherapy and check each participant's vitals. Telehealth is available for patients to participant via the internet.

Services provided are individual counseling, divorce counseling, faith based counseling, youth counseling, parent counseling, family counseling, grief/bereavement counseling, terminal illness counseling, child abuse counseling, crime victim/witness counseling, geriatric psychiatry, Cognitive Behavioral Therapy (CBT), and Trauma Informed Therapy. Areas of training or expertise include rural mental health - meeting farmers, ranchers, and their families where they are at, often times at their farm. Hours can be accommodated to fit the harvest/planting schedule; anxiety, depression, suicidal ideation, life transitions, farm succession, parenting strategies, communication styles, and trauma.
