Case Management for adults with intellectual/developmental disabilities who receive services through the HCBS Choices Waiver. Case Managers facilitate person-centered planning and assist people in applying to service providers and community resources. Serves residents of the Northeast and Southeast Regions in South Dakota. Conflict-Free Case Managers are not related to the individual, are not their paid caregivers, and are not financially responsible for the individual.

Circle of Friends is an inclusive program for people with intellectual and developmental disabilities. The main purpose is to provide the opportunity for spiritual development, worship, community service to others, and social settings for all abilities. During the school year Circle of Friends participate in Muffin Monday, baking muffins from scratch in the morning (at Open Heart and Canyon Lake UMCs), and handing them out to students walking home from school in the afternoon. Circle of Friends also gathers at Open Heart UMC on Tuesdays at 10:00am for refreshments, music, games, crafts, and spirituality during "Circle Time." Circle of Friends Sunday School is also available Sunday mornings at both Canyon Lake and Open Heart United Methodist Churches.

Black Hills Works is a community support provider that offers an array of programs to adults with intellectual and developmental disabilities. Services include residential, employment, volunteer and community services, specialty services, and art expression.

Family Support 360 (FS 360) helps people with intellectual and developmental disabilities and their families get the services they need to live in their own home and community as independently as possible. FS 360 is self-directed, which allows the person or their guardian to choose their staff and schedule based on their needs.

Family Support Service Coordinators act as an advocate for you and your family.
