Monument Health specialists work with providers and insurance companies to verify coverage and out-of-pocket costs. Staff is on call 24/7 to assist with any problems that may arise. Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

ICAP Housing Services provides the following programs: Rapid Rehousing and Homeless Prevention Programs: aims to help two specific groups - persons who are at great risk of homelessness and those who have become homeless. Services that can be provided include case management, security deposit and rent assistance. These services are aimed at helping participants to retain their housing, therefore avoiding homelessness, or to re-enter appropriate permanent housing as quickly as possible. Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Homeless Rent and Deposit: Helping homeless households get into housing. Must provide steady and reliable source of income. Applicant must agree to case management services.
