Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides support to Disabled Veterans and their families. Primary focus is to provide free transportation for veterans that have medical appointments at the Veterans Administration (VA) Hospital.

General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown location is also able to provide car seats for all ages when the purchase of a car seat is financial hardship.

Providing education, awareness, assistance, and shelter safety for victims of domestic violence, sexual assault, child abuse, and human trafficking. Services include shelter, food and gas vouchers, transit tickets, diapers, support groups, accompany to hospitals and court hearings, assistance with completing Crime Victim Assistance Applications and safety plans, and referrals to other support services.

Early Intervention Program that serves children from birth - 36 months with developmental delays or disabilities and their families. To schedule a developmental screen, contact the local service coordinator.

An event venue owned and operated by the City of Emery. The auditorium is available for a variety of events such as wedding receptions, community celebrations & banquets, meetings, workshops, & educational events, fundraisers & benefits, birthday parties & anniversaries, athletic events, practices, and indoor activities. Making Cent$ Financial Literacy Program: Focuses on women looking to get their finances in order. Sessions include local experts, individualizing the curriculum to participants' needs. Works with each person to set individual goals, provide encouragement, education and empowerment throughout their journey. ​Women's Leadership Program: Facilitates the development of female leaders by utilizing the experiences and skills of established women. Women in any career field can benefit from this intensive course, which combines a strong personal development focus along with valuable professional development experiences.

CES is an assessment and referral system to housing resources for those who are experiencing homelessness or at risk of homelessness. It is not a direct application for assistance; rather trained assessors gather information from households to assess needs and barriers to housing. Referrals are sent to providers in communities where vacancies are available. Households can access CES through a physical access point or calling toll free 1-800-664-1349 if a physical access point is not available in their local community.

Maintaining and constructing safer county roads. Highway and Road Maintenance on roads located within Yankton County. The Highway Department is responsible for the Unorganized Townships as well as County Roads. This does include bridges and culverts located on these roads. Also responsible for snow removal, mowing, and any other repair to the roads and/or right-of-ways.

Provides the following services - Advocacy: Teaches people with disabilities the skills needed to advocate for themselves. Independent Living Skills: Provides hands-on training in daily-living skills including cooking, budgeting, organizing, and more. Information and referral: Connects people with resources which can provide needed information to assist individuals to live independently. Peer counseling: Provides opportunities for people with disabilities to engage with others in small or large groups or on an individual basis. Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

Provides the following services: ​ ​Rental Rehabilitation Program - Up to $25,000 per rental unit; $100,000 maximum per project, depending on equity. Funds may be used to replace roofs, siding, insulation, exterior painting or installing energy efficient windows and doors. Funding may also be used for interior updates. ​Single Family Housing Rehabilitation Program - provides financial and technical assistance for housing rehabilitation needs including but not limited to roofs, doors, stairs, painting, insulation, handicap accessibility, windows, electrical, plumbing and furnace replacements. Maintenance repairs are not included in this program. Loan payments are only required when the home is sold or is no longer the primary residence. ​Emergency Mobile Home Repair Program - provides up to $5,000 to help pay for emergency repairs. Maintenance repairs are not included in this program. ​Neighborhood Revitalization Program - designed to remove dilapidated housing structures which are unsafe, and replace with new affordable housing options. New, owner-occupied residential structures constructed and eligible for income qualified households to purchase.

Services provided are coordination of all agencies within the jurisdiction to include public, private, and non-governmental organizations to ensure the protection of life and property in the event of threatened or actual natural disasters, terrorism, technological, or man-made events. ​Responsibilities includes develops and maintains the county plan for reacting to emergencies; during an actual emergency, the director coordinates the action; and trains other agencies to respond to emergencies. The field office then sends the case to a DDS for evaluation of disability The Social Security office may have a list of approved organizational payees.
