Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Delta Dental's mobile truck provides quality care to children from first tooth through age 21 who can't afford or access a dentist. Service is provided at no cost to the family, and no insurance is necessary. The Mobile truck is scheduled to visit Madison, SD June 9-12, 2025 and Flandreau, SD August 18-21, 2025.

The CASSP Dental Van provides dental care to under-served children 0 up until their 22nd birthday who meet the following requirements: Do not have a current dental home Have not seen a dentist in 2 years or more in the Aberdeen Area No child is turned away for inability to pay These children are those with the highest need for dental care and have the least amount of resources. This mobile dental unit provides preventive, diagnostic, and restorative services. No child is turned away for inability to pay. However, if their family or guardian have any type of dental insurance, it will be billed by the Delta Dental Mobile Program. An adult must accompany any child seen for dental care as consent must be given for the staff to work with the child. The Dental Van is a mobile unit that travels to communities around the state. This van is in Aberdeen only on specified dates which varies year by year.

​Almost every day, the Delta Dental Mobile Program's dental staff sees a child who is in pain because of untreated dental disease. These are children who, for a variety of reasons, have been unable to access adequate dental care. United Way & Volunteer Services partners with Delta Dental of SD to bring the Delta Dental Mobile Program to Yankton each spring to offer no cost dental care to children ages 0-21. **Pre-registration is required to participate. Registration deadlines vary.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.
