Provides nursing facility and short-term rehabilitation care in a home-like environment with individualized care plans. Services include but are not limited to 24-hour nursing care, wound care management, nutrition services, safety systems, pain management, recreational activities, in-house pet therapy, physical/occupational/speech therapy, and restorative therapy. Delta Dental's mobile truck provides quality care to children from first tooth through age 21 who can't afford or access a dentist. Service is provided at no cost to the family, and no insurance is necessary. The Mobile truck is scheduled to visit Madison, SD June 9-12, 2025 and Flandreau, SD August 18-21, 2025.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.
