Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

De Smet Dental offers a full range of family oral health services including: regular cleaning/hygiene appointments, examinations, fillings, crowns, bridges, root canals, oral dentures, and extractions.

Providing comprehensive dental services to pediatric patients. Services include: routine & preventative check-ups, dental hygiene instruction, dental fillings, general anesthesia, stainless steel crowns, and emergency.

Services include medical, dental, pharmacy (Prescription Assistance - Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS), Optometry, wellness center, diabetes program, behavioral health, physical therapy, full scale lab services, and radiology.

Services include 45 bed hospital, emergency room, Obstetrics, Pediatrics, Prenatal Care, Surgical, Dental, and Prescription Assistance. Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS.

The van travels to different communities, and is not an ongoing service (typically happens 1 or 2 times a year at a location).

Provides transitional and supportive housing services to veterans who are homeless or at risk of homelessness due to financial hardship, unemployment, addiction, mental health, or transition from jail. Services include connection to immediate food and shelter needs, job training, life skills development and education, justice system navigation for community reentry from jail, and financial support to prevent homelessness.

Provides the following services - emergency room, inpatient care, comprehensive primary care, Primary Care Medical Home (PCMH), behavioral health, adult and pediatric dental, eye clinic, radiology, laboratory, nursing, pharmacy, public health nursing, dietician, and telehealth (including behavioral health, internal medicine, and rheumatology).

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.

Horizon Health - Plankinton Dental provides full range of family oral health services including preventative (check ups, fillings) and restorative care (extractions and dentures). Accepts Medicaid patients.

Through its community-wide fundraising efforts, the Capital Area United Way helps to support 35 programs and services offered through 14 local United Way funded agencies. Supports Community Impact Programs that help improve the lives of people affected by issues such as health, income and education. Programs include Girls on the Run/Girls on Track serves girls in 3-8th grade and helps prepare them for a lifetime of self-respect and healthy living. Imagination Library provides free books to children, ages 0-5, who reside in Hughes and Stanley counties. Mobile Dental Program helps children who don't have access to dental care. Project H.E.R.O. provides milk tickets and emergency transportation tickets to students to ensure they are healthy and able to get to school on time, and ready to learn; and the Lion's Club Vision Screening. Serves as a connecting place for volunteer opportunities. Agencies share that information through a publication in the paper called Volunteer Connections, as well as in the high school newspaper. Also, helps connects groups looking for a project with agencies needing volunteers.

Dentistry With Heart offers one day of free dental services including a choice of extractions, cleanings or fillings for those 18 or older who can't get dental care elsewhere.

The VA Surgery / Specialty Care Service Line includes the following services: Orthopedics, ENT, Gynecology, Urology, general surgery, Opthalmology, Podiatry, Audiology, Optometry, Dental, GI Endoscopy, and Proctology.
