Delta Dental's mobile truck provides quality care to children from first tooth through age 21 who can't afford or access a dentist. Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

COVID Testing Option: ​Oyate Health Center Urgent Care 3200 Canyon Lake Drive 7:00am - 7:00pm (MT), 7 days a week Christmas Eve closes at 3pm (MT). Closed on Thanksgiving Day, and Christmas Day and New Year's Day. Oyate Health Center provides primary medical care, outpatient clinic, and urgent care. Oyate Health Center provides direct health services to Native Americans and provides contract health care services and referrals to those that are eligible within IHS requirements. This means that if a patient is enrolled in a federally recognized tribe or can prove lineage of descent of an enrolled tribal member; the patient may receive direct care services at Oyate Health Center. Prescription Assistance - Any Native American enrolled in a federally recognized tribe can receive prescription expense assistance at any IHS facility.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.

The Flandreau Santee Sioux Tribal Health Department is a full range ambulatory facility serving Native American people living in or near Moody County. Services include medical, dental, pharmacy (Prescription Assistance - Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS), Optometry, wellness center, diabetes program, behavioral health, physical therapy, full scale lab services, and radiology.

Offers free dental appointments for those 21 years old and younger.

Services include 45 bed hospital, emergency room, Obstetrics, Pediatrics, Surgical, Dental, and Prescription Assistance. Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS.

These are children who, for a variety of reasons, have been unable to access adequate dental care. United Way & Volunteer Services partners with Delta Dental of SD to bring the Delta Dental Mobile Program to Yankton each spring to offer no cost dental care to children ages 0-21. **Pre-registration is required to participate. Registration deadlines vary.

The Mobile Program includes a fully equipped mobile dental clinic to provide diagnostic, preventive, and restorative care across the state, as well as oral health education.
