Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Delta Dental Mobile Program | United Way and Volunteer Services of Greater Yankton

​Almost every day, the Delta Dental Mobile Program's dental staff sees a child who is in pain because of untreated dental disease. These are children who, for a variety of reasons, have been unable to access adequate dental care. United Way & Volunteer Services partners with Delta Dental of SD to bring the Delta Dental Mobile Program to Yankton each spring to offer no cost dental care to children ages 0-21. **Pre-registration is required to participate. Registration deadlines vary.

Service is provided at no cost to the family, and no insurance is necessary. The van travels to different communities, and is not an ongoing service (typically happens 1 or 2 times a year at a location).

Well child checkups, vaccinations, general pediatric health care. Mental Health: Offers outpatient counseling and psychiatric services, to both children and adults. Mobile Dental Program | Delta Dental

Provides oral health care to underserved children in South Dakota. The Mobile Program includes a fully equipped mobile dental clinic to provide diagnostic, preventive, and restorative care across the state, as well as oral health education. Oyate Health Center | Great Plains Tribal Leaders Health Board

COVID Testing Option: ​Oyate Health Center Urgent Care 3200 Canyon Lake Drive 7:00am - 7:00pm (MT), 7 days a week Christmas Eve closes at 3pm (MT). Closed on Thanksgiving Day, and Christmas Day and New Year's Day. Oyate Health Center provides primary medical care, outpatient clinic, and urgent care. Additional outpatient services that are provided on site include optometry, pharmacy (Prescription Drug Take Back Site, blue drop-box by pharmacy), laboratory, radiology, dental, physical therapy, podiatry, and social services. Oyate Health Center has 2 sweat lodges. Please call for details. Oyate Health Center provides direct health services to Native Americans and provides contract health care services and referrals to those that are eligible within IHS requirements. This means that if a patient is enrolled in a federally recognized tribe or can prove lineage of descent of an enrolled tribal member; the patient may receive direct care services at Oyate Health Center. Prescription Assistance - Any Native American enrolled in a federally recognized tribe can receive prescription expense assistance at any IHS facility.
