The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Provides personal care and home care services to veterans. Includes personalized care plans, medication management, mobility support, nutritional planning, home safety assessments, and specialized services designed to meet unique needs.​

Provides long-term nursing facility care and short-term rehabilitation care with 24/7 personalized nursing services. Services provided include licensed therapies, social and spiritual activities, nutrition program, care planning, and more. Also provides memory care services, outpatient rehabilitation services, respite care, and adult day services.

Provides a variety of home health service including nursing services (nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.), personal care (bathing, dressing, feeding, ambulation & mobility, incontinence care etc.), homemaking/companion care (laundry, housekeeping, meal preparation, etc.), and structured family caregiving (a program which provides a financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver).

Providing in-home care to those who need assistance with activities of daily living. Services include medical assistance such as in-home memory care, diabetes care, arthritis care, respite care, Alzheimer's and Dementia care, hospice support, chronic illness care, and 24-hour care. They can also assist with personal care (i.e. bathing, dressing), companionship, assistance with daily activities (i.e. meal preparation, light housekeeping), and transportation. These services are designed to enhance the quality of life and promote independence for clients in their own homes or community settings.

Provides Memory Care with communities designed to include higher staffing levels. Services include 24-hour staffing, secure environment, dietary services, group and individual activities, assistance with personal cares, medication administration, and laundry and housekeeping services. Also provides adult day services (designed for those affected by Alzheimer's disease or other forms of dementia) and respite care.

Offers complimentary in-home care for family caregivers of individuals living with dementia, fully funded by Medicare. The program offers $2,500 toward professional home care services to be used through the year.

Provides skilled nursing facility care tailored to short-term rehabilitation and long-term care services. Also provides respite care services and a secured memory care unit with staff trained to care for those living with Alzheimer's and Dementia.

Offers independent living, assisted living, and memory care services offering a range of assistance with activities of daily living for aging residents, including those who have been diagnosed with dementia and Alzheimer's. Respite Stays are available in most locations except in Rapid City.

Supporting caregivers that help their Medicaid-eligible loved ones at home by providing an expert care team, emotional support, and financial assistance.

Offers a family-centered approach to care and is committed to providing only the most appropriate services based on regular assessments of your needs. Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Provides in-home services including medical care (wound care, medication administration, medical tests, skilled nursing services, and hospice care), companion care (escort/transportation, meal preparation, protective supervision, respite care, shopping assistance), personal cares (bathing, dressing and grooming, body care, assistance with doctor's appointments, feeding assistance, positioning, standby assistance, total assistance, and ambulation assistance), and homemaker services (cleaning assistance and other light housekeeping).

Home Health can help patients make a good transition from hospital to home, or receive the help and care they need at home. Services include physical and occupational therapy, medication management, wound care, IVs and central lines, personal care, light housekeeping and meal preparation.
