The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Hospice services use a team approach of physicians, nurses, social workers, home health aides, volunteers, chaplains, family members and others. Hospice strives to provide quality medical care, pain and symptom management, and emotional and spiritual support tailored to help ensure a higher quality of life. Nursing visits are made on a regular schedule and an on-call 24-hour basis as needed. Additional services include inpatient and physician-directed services, interdisciplinary, integrated services, and bereavement counseling.

The Family Support 360 Program (FS 360) provides people and their families with services and supports to meet their unique needs. The FS 360 Program is self-directed, which means the person and their family choose the services, the staff who provide them, and a schedule based on their needs. FS 360 helps people and their family receive state funded services and other community services, together with natural supports (extended family members, friends, neighbors, church congregations, and community organizations). Each person receiving FS 360 services chooses a Family Support Service Coordinator who will help you find state funded services, community services, and natural supports. Once these services and supports are found, the Family Support Service Coordinator will help control a budget for the services. Most importantly, your Family Support Service Coordinator will advocate for you and your family. Services & Supports include Service Coordination, Environmental Accessibility Adaptive Equipment, Vehicle Modifications, Companion Services, Supported Employment, Personal Care, Respite Care, Special Medical Adaptive Equipment and Supplies, Nutritional Supplements, and Specialized Therapies.

Amenities include private rooms with guest beds, patios or decks, family gathering spaces, shared kitchen, guest room, chapel, library, outdoor walking areas, wireless internet, and a spa with accessible bathing options. Visitors are welcome per patient discretion; mail, flowers, and Mylar balloons accepted. Levels of Care Offered: acute care (skilled nursing for patients with rapidly changing conditions), respite care (up to 5 days for caregiver relief), and residential care (for patients unable to stay at home).
Provides adult day services, which includes personal care, frequent outings, entertainment, exercise, outpatient therapy, live music/dancing, hair salon, and arts/crafts. Also provides free monthly education on aging related topics and support groups. For Medicaid-qualified guests, community health worker services are available.

The eligibility categories are: Developmental delays for children under 3 years Developmental disabilities Chronic medical conditions for children up to age 18 Serious emotional disturbance Severe and persistent mental illness Traumatic brain injury, prior to age 21. The respite care program is also available to post-adoptive families, referred by Adoption Agency, with or without special needs, and foster parents who are referred by the SD Department of Social Services.
