A 24-hour source of support and information to cancer patients and their families. Reasons for calling the Navigation Center can range from managing symptoms, to finding resources for emotional support, to getting help for loved ones impacted by cancer. Anyone can call the Navigation Center - they do not need to be an Avera patient. Calls are answered by registered nurses and social workers. The Avera Cancer Institute has a registered dietician that can assist cancer patients with eating problems and related nutritional concerns, and offers several cancer support groups (in the Sioux Empire area) including A Time To Heal for All Cancers, Cancer Caregiver Support Group, Look Good, Feel Better, Men's Cancer Support Group, and To Be Well.

Services include program education and support and personal attendant services. - MONEY FOLLOWS THE PERSON - Providing options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

A social group for families who have a child with a disability or rare disease to meet in a setting where kids can play with their peers and parents to connect with each other. Families can also connect in a private online community to ask questions, get answers and recommendations, as well as a safe space to share their experiences. Grief Counseling Services Monument Home+ Home Health and Hospice offers individual, couples, and family grief counseling to the immediate family members of the patients who were on our services. Counselors are professional social workers with specialized training and experience in grief and loss. Since people grieve differently, counseling is offered to meet the distinctive needs of different age groups and types of loss. Call and schedule an appointment. These groups bring people together who have experienced loss and provide a safe environment to share experiences and learn from one another. Helps families and their support systems find hope and healing after the loss of a child at any age. Provides encouragement, compassion and support through retreats, education and small groups. The groups provide a way to connect with other families who are walking a similar journey. Different from other retreats, Healing Hope hosts only 1 family at a time (in a guest house on on 5-acre property), creating a unique one-on-one experience. The facilitators walk alongside the families, not as counselors, but as peers that have been on a similar journey. Healing Hope also appreciates the opportunity to educate the community by sharing personal stories of grief and life lessons with different groups, churches, schools, etc.

 Working to provide connection, comfort, and hope to families who have experienced the unexpected loss of an infant. They provide care packages to those families and a network of connections to others who have experienced a similar loss. A Run for Their Lives! 5K run/walk event is held annually on Mother's Day. Participate in-person in Spearfish, SD, or virtually from anywhere! This event is a great way to honor the memory of a child with a community of supporters. Also works to provide SIDS and safe-sleep awareness and education as well as funding for SIDS-related research. Operations are home-based.

Education Public Awareness: Hosts education days each year to bring in guest speakers to discuss different topics about HD such as discoveries about the disease, new and promising research, discoveries about the progression of the disease and resources available to families and patients. Outreach: Local chapter support available online through their Chapter website and social media channels such as Facebook. By keeping a constant stream of information online, patients and families can find support and information at all hours of the day or night. Support Group Hosts a monthly support group meeting for families who are affected by HD twice a month. This group reviews caregiver support, discusses genetic testing, care options and serves as a general sounding board to support one another through the journey of this disease.
