Programs and Services | Dysphagia Outreach Project, The

Collects and distributes dysphagia supplies to individuals in need, establish a dysphagia awareness advocacy program, provide education services relating to appropriate prescription and use of dysphagia supplies, and conducting research to improve the equitable distribution of dysphagia supplies to people in need and improve the autonomy of people with feeding and swallowing disorders. ​The Dysphagia Food Bank provides tangible assistance to low income individuals with dysphagia across the lifespan by providing access to thickeners, pre-modified foods, therapy tools, oral care supplies, and adaptive equipment. The food bank is available to all 50 states. Income qualifications apply.

Brookings Area Brain Injury Support Group | Brain Injury Alliance of South Dakota

Any individual who has had a brain injury is welcome to attend. The support group is also available for family members. Brain injuries can be obtained from accidents, falls, multiple concussions, tumors, stokes, lack of oxygen, epilepsy and Parkinson's, to name a few. The brain injury may be a recent injury or one that has been acquired for many years.

Monument Health Home+ Home Health and Hospice provides educational, supportive 12-week grief support class/group. These groups bring people together who have experienced loss and provided a safe environment to share experiences and learn from one another.

Peer counseling: Provides opportunities for people with disabilities to engage with others in small or large groups or on an individual basis. Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Leukemia & Lymphoma Support | Leukemia & Lymphoma Society

Personalized Support: Speak one-on one with a highly trained oncology nurse or social worker who can assist you through blood cancer treatment, financial and social challenges, and give accurate up-to-date disease treatment and support information. First Connection: For those who have recently diagnosed with cancer, the Leukemia and Lymphoma Society has provided the opportunity to speak with someone who's been through treatment and learned how to manage the same disease. The Patti Robinson Kaufmann First Connection Program is a peer-to-peer program that matches newly diagnosed patients and their families with trained volunteers who have been touched firsthand by a blood cancer and shared similar experiences. Online Chats: Live, weekly online chats provide a friendly forum to share experiences and chat with others about anything from the initial phase of diagnosis to treatment and survivorship. Each chat is moderated by an oncology social worker and is password-protected. The online chats are not a replacement for psychotherapy or support groups. Patient Financial Assistance Programs (visit website for more information): Programs may cover copays, other medical expenses, urgent need for non-medical expenses, treatment-related travel expenses, clinical trial related-travel expenses and general financial support. Brain Injury Caregiver Support Group | Severe Brain Injury Caregivers

Provides a support group for anyone who cares for someone with any of the following: wounds to the head or head injury, motor vehicle or bicycle accidents, falls, partner violence or physical abuse, lead poisoning, drug overdose, long-term neurocognitive or neuropsychiatric complications from stroke, birth defects, and any illness or disease. Provides an online community that provides peer-to-peer support that is tailored to meet the special needs of caregivers to persons with moderate-to-severe brain injuries or disease. This group believes there is always hope and will to will never give up on survivors because mutual support and sharing are an important part of the caregiving process.

Health Education Workshops | SDSU Extension

Building Better Caregivers: workshop designated for family caregivers of people living with dementia, Alzheimer's, brain injury, other conditions that affect memory, and those who can no longer live at home independently. This is a virtual training. Surviving and Thriving Workshop: workshop designed for cancer survivors and caregivers to improve self-management skills. Chronic Disease Self-Management Workshop: workshop designed to improve the self-management skills of people living with ongoing health issues. Chronic Pain Self-Management Workshop: workshop designed to help participants live a healthy life with chronic pain by managing their symptoms. This is a virtual training. Diabetes Self-Management Workshop: workshop designed for people with prediabetes, type 2 diabetes, a family history of diabetes, and caregivers. Walk with Ease Workshop: a 6-week community-based walking program designed to engage participants to maintain a successful waking program. Workshops include health education, stretching, and group or self-guided walking. Enhance Fitness: group exercise program that helps older adults improve strength, balance, flexibility, and cardiovascular health. Tai Chi for Arthritis and Fall Prevention: exercise program designed to improve balance, reduce pain, and lower the risk of falls. Aging Mastery Program (AMP): 10-week program that helps older adults build their own playbook for aging well.

Better Choices Better Health - South Dakota (BCBH-SD) | SDSU Extension

A statewide community resource that offers chronic disease self-management education workshops to help adults living with ongoing physical and/or mental health conditions. The workshops also help caregivers understand how healthier choices can improve quality of life, boost self-confidence, and inspire positive lifestyle changes. These workshops teach skills and tools to use that can help you manage symptoms, improve the quality of day-to-day living, and reduce healthcare costs. Workshops are offered in 6 weekly sessions and are free to attend.
