Avera Medical Group Brookings is a multi-specialty medical group providing clinic laboratory, digital mammography, imaging and x-ray, 3D mammography, ear, nose and throat, family practice, general surgery, infusion/chemotherapy, internal medicine, obstetrics & gynecology, orthopedics, outreach specialists, pediatrics, physical therapy, dermatology, podiatry, and urology. A Coordinated Care Team is available to help identify resource support for community and primary healthcare needs; facilitation and connection to long-term counseling services to meet mental health needs, and clinic based counseling for interim or short-term support needs.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides individual, couples/marriage, and family counseling. Topics covered include depression, anxiety, obsessive compulsive disorder (OCD), attention deficit/hyperactivity disorder (ADHD), adjustment disorders, bipolar, post traumatic stress disorder (PTSD), child sexual abuse, domestic violence, marriage issues, spiritual issues, women's issues, church leadership abuse issues, Asperger's, social skills, self-esteem, bullying, and individuals whose spouses have addiction issues.

Outpatient Services - Individual therapy (for mental health or substance use), Family therapy, Group therapy, Medication management, Psychological assessments and testing, Commitment evaluations, Nursing home consultations, and 24-hour a day crisis intervention via crisis line. Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Provides outpatient mental health services to those 12 and up. Works with a variety of disorders/concerns: anxiety, mood disorders, depressive disorders, trauma related disorders, adjustment disorders, personality disorders, crisis intervention, and/or marital/family distress. They become part of a healthy, safe community of support that provides compassionate assistance while they address pressing legal, medical and emotional needs. Services and Support - Women receive addiction counseling, job skills/GED classes, emergency counseling and transportation. Services wil also include a crisis hotline, clothing, food and hygiene items. Self Development - Women learn life skills such as communication, conflict resolution, understanding boundaries, understanding trafficking and trauma, and making safe choices to help them live independent lives. Cannot accept anyone who is still using drugs or alcohol. If the victim is still using substances/has addiction issues, they must attend treatment prior to receiving services.
