Aimed at helping people with a severe and persistent mental illness to live successfully in the community. This program stresses outreach to consumers and the provision of services according to the individual consumer's needs, and is available through Dakota Counseling Institute's Pathway program located in a separate building beside the main office in Mitchell. Each consumer is assigned a case manager who, with the active participation of the consumer, designs an individualized treatment plan outlining the various services which will be provided to reach individualized treatment goals. Examples of services provided are case management, medication management, crisis intervention housing assistance, linkage to appropriate community services, and telehealth services.

Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services.
