The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Provides assisted living and independent living care. Assisted living services include but are not limited to medication management, assistance with activities of daily living, and maintenance. Independent living services include maintenance and 24/7 on-call help. Also provides adult day services and respite care.

A 50-bed certified nursing home offering nursing facility care with 24-hour a day nursing care. Physical therapists evaluate and design exercise programs and daily and evening activities are scheduled to meet social needs. A private dining area is available for family gatherings. Financial services, social services, and pastoral care are available. Also provides adult day services.

Assisted living services provide a living option for those who want to live independently while benefiting from having access to help with activities of daily living. Services provided include help with daily activities, laundry and housekeeping, home maintenance, medication management, spiritual, wellness, nutrition, and recreational programs. A variety of services are available that may be included in the monthly fees such as assistance with general housekeeping, maintenance, optional meal programs, scheduled activities, transportation services, wellness programs, social interaction, and nearby staff. Provides comprehensive supports (i.e. supported employment, residential, community engagement, adult day services, representative payee services, etc.) for adults with intellectual/developmental disabilities.

CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders. Components of the program include residential services, independent living training, educational services, and therapeutic and psychiatric services (individual and group therapy).

A 31-bed long-term care facility providing nursing and assisted living care. Also offers respite care, adult day services, hospice care, short-term rehabilitation, and therapy services.

Supports individuals in the community with developmental delays by providing residential services, employment services, day programming, and nursing.

Special services include a Memory Care Unit and Adult Day services.

Depending on the level of care need, services include but are not limited to 24-hour nursing care, therapy services, Alzheimer's special care, hospice services, dietary services, and adult day/respite care.

Day Services are provided for people with intellectual and developmental disabilities.

Services include 24-hour staffing, 24-hour nursing services, emergency pendent system, dietary services, medication management, assistance with personal care, housekeeping and laundry, activities and outings, and some transportation and coordination.
