Long Term Services and Supports provides opportunities to enhance the quality of life for older adults, adults with disabilities, and their caregivers through home and community based services. Services Include: home care services to help adults remain in their home, safety and help moving around, nutrition and meals, Senior Health Information and Insurance Education (SHIINE) providing individuals information about benefits they can receive through Medicare, caregiver support, Structured Family Caregiving, residential living services, Ombudsman program providing advocacy and education for residents, family, and staff in long term care settings, and adult protective services providing interventions to protect vulnerable adults from abuse, neglect, and exploitation.

A community support provider offering services to individuals with disabilities in the areas of residential services, day services, medical services, personalized support plan, to help with attaining desired dreams and goals in the areas of home, work, and community; and the Career Advantage program.

Provides long-term nursing facility, short-term rehabilitation, assisted living, and independent living care. Nursing facility care provides long-term nursing facility and short-term rehabilitation care with 24/7 personalized nursing care. Services provided include licensed therapies, social and spiritual activities, nutrition program, and care planning. Assisted living services provide a living option for those who want to live independently while benefiting from having access to help with activities of daily living. Services provided include help with daily activities, laundry and housekeeping, home maintenance, medication management, spiritual, wellness, nutrition, and recreational programs. Independent living services provide a living option for individuals looking for maintenance-free living. A variety of services are available that may be included in the monthly fees such as assistance with general housekeeping, maintenance, optional meal programs, scheduled activities, transportation services, wellness programs, social interaction, and nearby staff. Also offers memory care, outpatient rehabilitation services, and adult day services.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.

Day Services are provided for people with intellectual and developmental disabilities.

Provides Memory Care with communities designed to include higher staffing levels. Services include 24-hour staffing, secure environment, dietary services, group and individual activities, assistance with personal cares, medication administration, and laundry and housekeeping services. Also provides adult day services (designed for those affected by Alzheimer's disease or other forms of dementia) and respite care.

Provides assisted living and independent living care. Assisted living services include but are not limited to medication management, assistance with activities of daily living, and maintenance. Independent living services include maintenance and 24/7 on-call help. Also provides adult day services and respite care.

Provides services and supports to people with Intellectual and Developmental Disabilities including residential services, day program, vocational services, supported employment, and health care coordination.

Provides individualized care for elderly individuals with dementia or other cognitive impairment as well as provides support for their caregivers.
