A 50-bed certified nursing home offering nursing facility care with 24-hour a day nursing care. Physical therapists evaluate and design exercise programs and daily and evening activities are scheduled to meet social needs. A private dining area is available for family gatherings. CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders. Components of the program include residential services, independent living training, educational services, and therapeutic and psychiatric services (individual and group therapy).

Supports individuals in the community with developmental delays by providing residential services, employment services, day programming, and nursing.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.
